Objective:According to the theory of Chinese Medicine,Delayed Onset Muscle Soreness(DOMS)belongs to "damage of Jin caused by too much exercise",that is,soft tissue discomfort caused by excessive exercise,which belongs to "Jin" overwork but not yet reach"injury of Jin"(Strain).From the etiological point of view,DOMS occurs after an unaccustomed eccentric contraction exercise,which is not due to internal or external causes.It is caused by overwork of muscles,and "Jin lose nourishment".As a treatment method of traditional Chinese medicine,massage can effectively relieve the symptoms of DOMS through relaxation and stimulation.Repeated eccentric contraction is a relatively new rehabilitation training to prevent DOMS in recent years.This study uses innovative Chinese Medicine’s massage combined with repeated Eccentric contraction as a joint intervention program to observe the clinical effect of combining two different types of DOMS intervention methods,and explore possible mechanisms for preventing and controlling DOMS through clinical research and animal experiments.Methods:(1)Clinical research:a clinical controlled trial,recruiting students from Beijing University of Chinese Medicine who meet the research requirements on the Internet.Each volunteer’s bilateral gastrocnemius muscles were randomly divided into an intervention group and a control group,with a sample size of 1:1.The modified Step Test exercise was used as a large amount of eccentric contraction to induce DOMS.Shortly after exercise,the gastrocnemius muscle of the intervention group was treated with standardized massage,and the interventional side gastrocnemius muscle exercise was arranged at a fixed time according to the same standardized rehabilitation training;control group The gastrocnemius muscle does not intervene.During the study period,neither of the gastrocnemius muscles exercised vigorously.Volunteers complete the pain-related scales after receiving training,and the relevant data of volunteers who successfully induced DOMS will be included in the study.The researchers measured the Pressure Pain Threshold of a fixed position of the gastrocnemius muscle of volunteers at the time of the beginning(no exercise)and when they finished the research(about 48 h after the beginning),after excluding unsuccessful and other shedding samples,a paired sample test was conducted to observe the joint intervention in suppressing the pain during the DOMS of muscle after a large amount of eccentric contraction exercise.(2)Animal experiment:In the control experiment,16 male 10-week-old Wistar rats were randomly divided into an intervention group and a control group after receiving the same treadmill exercise.The intervention group repeated the eccentric contraction exercise once a day with a small amount of exercise,and the control group did not exercise.On the 3rd and 7th days after modeling,4 rats were randomly selected from each group for sampling.The sampling materials included rat arterial blood serum and rat distal quadriceps tissue.Measure the concentration of rat serum lactic acid and lactate dehydrogenase with a kit,analyze the morphological structure of rat skeletal muscle from different microscopic levels with the aid of optical microscope and electron microscope,and analyze the results of repeated eccentric contractions on the quadriceps of rats after large-scale exercise according to the above results.The intervention effect of muscle injury recovery,and explore the mechanism of related results.Results:(1)Clinical study:① All volunteers were included in the study.The Pressure Pain Threshold of gastrocnemius in the intervention group(side)did not decrease significantly compared with the value collected when they entering the group(P>0.05);the Pressure Pain Threshold in the control group(side)when exiting the research significantly(P<0.05)lower than that at the time of the beginning,the average value of decrease was 4.16 N.②The Pressure Pain Threshold of gastrocnemius in the male intervention group(side)did not decrease significantly when entering the group(P>0.05).The Pressure Pain Threshold of the gastrocnemius in the control group(side)was significantly lower than that when entering the group(P<0.05),and the average value of decrease was 5.57 N.③There was no significant change in the Pressure Pain Threshold of the gastrocnemius between the two points of time in the female’s both group(P>0.05).(2)Animal experiment:① The serum markers LA and LDH of two groups of rats had no statistically significant difference on the 3rd and 7th day after modeling(P>0.05).②It can be seen under the optical microscope that the control group samples showed more interstitial cells than the intervention group 3 days after modeling,and the control group samples showed more interstitial cells than the intervention group 7 days after modeling.No obvious inflammatory infiltration was observed in the H-E stained sections of the quadriceps muscles of the two groups of rats,③ Visible under the electron microscope.The Z-line distortion,blurring,and fracture of the control group samples on the 3rd day after modeling were more obvious than the intervention group;the number of mitochondria in the control group was less than that in the intervention group on the 7th day after modeling,and the Z line of the two groups of samples recovered to a relatively neat and clear state.Conclusion:(1)The joint intervention program of Chinese Medicine’s massage combined with active rehabilitation training can significantly prevent the decrease of Pressure Pain Threshold of gastrocnemius muscle caused by DOMS after a large amount of eccentric exercise.(2)DOMS belongs to the category of "Jin lose nourishment" in Chinese Medicine,and its severity is not as good as "injury of Jin" and it can heal itself.The basic syndrome changes in the course of the disease are as follows:"Qi Xue Yu Zhi"→Jin lose nourishment(Qi Xue Liang Xu)→"Qi Xue Tiao He".It is speculated that the mechanism of the therapeutic effect may be:①The massage technique can effectively disperse Qi and Xue which sticked in there,and can promote the cycling of Qi and Xue.② Active rehabilitation induced much Qi and Xue gathered in gastrocnemius then help it recover faster.(3)In the animal experiment,the recovery of skeletal muscle in the intervention group was better than that in the control group at the two points of time.The researchers believe that even continuous repetitive eccentric exercise,which is far less than the amount of modeling exercise,can show adaptation,even in 3 days.Repeated eccentric contraction does not increase the ability to decompose LA by significantly increasing LDH activity in rats.The amount of exercise that induces acute changes in the ultrastructure of the rat quadriceps muscle is not sufficient to cause long-term ultrastructure changes.It is speculated that the DOMS of the human quadriceps muscle will not lead to knee osteoarthritis by causing irreversible muscle damage.(4)This study provides professional athletes and the public with reference to the treatment of DOMS,and provides a preliminary experimental basis for future clinical research.
