Pre-hospital medical first aid means that the emergency center(station)and the network hospital(hereinafter referred to as the first-aid network hospital)which undertake the task of pre-hospital medical emergency treatment are dispatched according to the unified command,and before the patient reaches the medical institution for treatment,Outside the medical institution to carry out on-site rescue,transshipment of emergency treatment and guardianship of the main medical activities.Pre-hospital medical first aid is responsible for daily emergency service and emergency medical rescue,major accident and medical guarantee for major activities.Time is life.Most critical patients have a sudden onset.How can we rush to provide effective basic life support to critical patients and quickly transfer them to the corresponding hospital for treatment so as to save the lives of the patients as far as possible?It is the task of the pre-hospital first aid institution and the public management duty of the government.It is very important for the government to provide timely and effective pre-hospital emergency service for the members of the society.Dalian pre-hospital first aid is built and managed according to the principle of territoriality.There are many kinds of pre-hospital first aid modes such as independent type,command type and relying type,which are also the main pre-hospital emergency service modes in various parts of China.We adopted the methods of literature research and current situation investigation,through investigating the present situation of first aid in Dalian,and analyzing the development of domestic pre-hospital first aid,we pointed out that there are insufficient stations and ambulance vehicles,difference of operation ability and shortage of emergency personnel pre-hospital first aid,weakness inlegal system,low ability of command and dispatch,and so on.We analyzed the progress of pre-hospital first aid in foreign countries and regions,summarized the advanced experience,and putforward that the government should assume the responsibility of pre-hospital first aid service as one of the public health services,perfect the legal system of pre-hospital first aid,and increase government input,strengthen management,strengthen team building,set up modern dispatching command system,construct socialized emergency system.It is necessary to strengthen the responsibility of the government,set up the overall planning,respond quickly,equitably and efficiently run the system of pre-hospital emergency service,and meet the needs of the urgent and critically ill patients before hospital and the diversified needs of the ordinary patients.We clarifies the responsibility of the government in the pre-hospital first aid,the government should fulfill the responsibility of the public product provider,increase the resource input,standardize the management,and operate efficiently so as to ensure social members of the community to have equitable access to pre-hospital first aid.Strengthen the responsibility of government public administration,while safeguarding basic services,properly play the role of the market,guide social forces to provide non-emergency transshipment services,provide diversified services for patients,and rationally determine the price of services,In order to improve the level of medical security,the pre-hospital first aid of critical and severe patients should be included in the payment range of medical insurance.Gradually establish a sustainable and socially integrated pre-hospital first aid system.
