ObjectiveTo observe the effect of health education based on the integration theory of behavior change on the fear of falling patients after hip replacement,and to explore the effect of health education on the fear of falling,ability of daily living,hip function and quality of life of patients after hip replacement.Thus to provide practical and scientific basis for the application and promotion of health education based on the integration theory of behavior change in the fear of falling hip replacement patients.MethodsThis is an experimental study.Patients who underwent hip replacement for hip fracture in traumatic surgery department in a tertiary Class A hospital in Tianjin from June 2019 to December 2019 were selected as the research subjects.Those who met the inclusion criteria and volunteered to participate in the study with informed consent were randomly divided into groups according to their ward and 50 cases were divided into the test group and 50 cases in the control group.After enrollment,the patients in the control group received routine health education,and the patients in the test group received health education based on the integration theory of behavior change.The intervention duration of the two groups was equal for a total of 12 weeks.Patients' fall efficacy scale,Barthel index rating scale,Harris hip function assessment scale and quality of life scale were evaluated at 1 day preoperative,and 2 weeks,4 weeks,and 12 weeks after surgery.Result 1.Baseline data shows that there is no statistically significant difference in baseline data such as age,gender,education level,living conditions,and falls of patients in the past year(P>0.05).One day before surgry,there is no statistically significant difference between the scores of fall efficacy scale,Barthel index rating scale,Harris hip function assessment sale and quality of life scale for the patients of two groups(P>0.05),showing that the two groups are balanced and comparable.2.Effect after intervention(1)Intervention effect of fall efficacyThere are statistically significant differences between the text group and the control group in the total scores and the indoor activity scores of fall efficacy scale after 2 weeks of intervention(P<0.05).Compared the total scores and dimensions scores of fall efficacy sale of the test group with the control group after four and twelve weeks of intervention,the differences are all statistically significant(P<0.05)and the scores of the test group are all higher than the control group over time.It shows that health education based on the integration theory of behavior change has certain advantages in reducing patients' fear of falling.(2)Intervention effect of Barthel index rating scaleThere is a statistically significant difference between the test group and the control group after 2 and 4 weeks of intervention(P<0.05).Although the scores of the Barthel index rating scale of the test group after 12 weeks of intervention are higher than the control group,the difference is not statistically significant(P> 0.05).It shows that health education based on the integration theory of behavior change can improve the daily living ability of patients at 2 and 4 weeks after surgery.(3)Intervention effect of hip functionExcept for deformity and activity,the differences in the other dimensions scores and total scores between the test group and the control group are statistically significant after 2 weeks of intervention(P<0.05).Except for deformity,the differences in the other dimensions scores and total scores are different after 4 weeks of intervention.Except for deformity and pain,the differences in the other dimensions scores and total scores are statistically significant after 12 weeks of intervention(P<0.05)and the test group scores are higher than the control group over time.It shows that health education based on the integration theory of behavior change can improve patients' hip joint function and improve their daily activities ability.(4)Intervention effect of quality of lifeThe differences in each dimension of quality of life between the test group and the control group are statistically significant after 4 weeks of intervention(P<0.05).The differences in vitality and mental health,physiological functions,energy,social functions,and mental health between the test group and the control group are statistically significant after 12 weeks of intervention.It shows that health education based on integration theory of behavior change can improve the quality of life of patients with hip replacement.Conclusion 1.This study confirms that health education based on the integrated theory of behavior change can reduce the fear of falling in patients with hip replacements,improve exercise compliance,and enhance confidence in disease recovery.2.Health education based on integration theory of behavior change can effectively improve patients' daily living ability,hip function and improve quality of life.
