Objective:To explore the effect of self-management education based on the Integrated Theory of Health Behavior on self-management,lung function,and quality of life of patients with chronic obstructive pulmonary disease,The aim of this study was to provide further reference and evidence-based basis for the application of the patients with COPD health management.Methods:According to the inclusion and exclusion criteria,a total of 156 stable patients with COPD were included in our study.The patients with COPD admitted to hospital during from Octor,2017 to December,2017 were as control group and during from January,2018 to March,2018 as experimental group.The number of patients with COPD who completed the study was 144,including 71 in the control group and 73 in the intervention group.The patients in control group received three times of hospital routine health education and outpatient follow-up.In addition to the routine healthy education and follow-up,the self-management education based on the integrated theory of behavior change was also performed for the cases in experimental group.The self-management ability of the patients was estimated by self-management scale table specific for patients with COPD,while the world health organization quality of life measurement scale(WHOQOL-BREF)was applied for estimation of life quality.The self-management level,lung function status and quality of life of the two groups of patients with COPD were compared before intervention,1 month after intervention,3 months after intervention and 6 months after intervention.Statistical analysis was performed using SPSS 22.0 software to explore the effect of self-management education based on behavioral change integration theory on self-management ability,lung function level and quality of life of patients with COPD.Results:1 There was no significant difference in the general data characteristics between the two groups(P>0.05).2 There was no significant difference in the total score of self-management and the scores of each dimension between the two groups before intervention(P > 0.05).1-month-after-intervention,there was no significant difference in the total score of self-management,daily life management,emotional management and self-efficacy between the two groups(P > 0.05).However,the total score of self-management,symptom management,emotional management and self-efficacy in the 3-month-after-intervention group were higher than those in the control group(P<0.01).And 6-month-after-intervention,the total score of self-management and the scores of symptom management,daily life management,emotion management and self-efficacy management in the intervention group were higher than those in the control group(P<0.01).3 There was no significant difference in FEV1/FVC between the two groups before intervention and 1-month-after-intervention(P>0.05).However,the FEV1/FVC i n 3-month-after-intervention group and 6-month-after-intervention group were higher th an that in the control group(P<0.01).4 There was no significant difference in the total score of quality of life and the scores of each dimension between the two groups before intervention(P>0.05).However,in the intervention group,the scores of the total quality of life,psychologi cal field and environmental field of the intervention group were higher than those of the control group(P<0.05 or 0.01).The total quality of life and psychology of the intervention group were 3 months after the intervention.The scores in the field,soc ial relations and environmental field were higher than those in the control group(P<0.01).The scores of the quality of life and the three dimensions of the psychologi cal,social and environmental fields were higher in the intervention group than in the control group.(P<0.01).Conclusion:Self-management education based on behavioral change integration theory can significantly improve the self-management ability of patients with COPD,effectively improve the patient's lung function,effectively improve the patient's quality of life,and help patients establish and maintain good health behavior,thereby alleviating the disease and improving the prognosis.
