| Objective: To investigate the necessity of using traditional warm-needle therapy for lumbar protrusion in clinical practice,and to compare the effects of traditional warmneedle therapy and common acupuncture on lumbar protrusion.Summarize the operational requirements of traditional warm-needle therapy for the treatment of lumbar protrusion in the grassroots community,and to provide a safe and effective method for clinical treatment of lumbar protrusion.Methods: Between July 1,2017 and December 31,2018,a total of 84 patients with lumbar protrusion who met the conditions in the outpatient clinic of Liu Zao Community Health Service Center in Pudong New Area,Shanghai,voluntarily signed informed consent.They were divided into treatment group and control group randomly.The treatment group was treated with traditional warm needle therapy,and the control group was treated with ordinary acupuncture.The treatment was performed 3 times a week,and 2 weeks as a course.Pain index(VAS score)was used to evaluate the pain degree of patients before and after treatment.The total score of the symptom(JOA score)was used to evaluate the symptoms of lumbar protrusion,and the dysfunction index score(ODI score)was used to evaluate the effect of low back pain on patients’ daily life.Macnab efficacy assessment criteria were used to evaluate efficacy.During the relatively concentrated time period,data were collected for the treatment of patients with cold-stagnation and stagnation of blood stasis and lumbar stenosis,including peak temperature,temperature increased,time to maximum temperature,and the duration of the raised temperature.Recording was performed three times and statistical analysis was performed using SPSS25.0 software.Results:(1)There were no significant difference in on pre-treatment syndrome differentiation,division,imaging,segmentation,VAS score,JOA score,ODI score between the two groups.(2)The VAS score(P<0.001),JOA score(P<0.001)and ODI score(P<0.001)of the two groups were significantly improved compared with before treatment indicating that both therapy were effective.Meanwhile,the improvement of the treatment group over time was better than that of the control group.(P<0.001).(3)The efficacy of the modified Macnab was compared between the two groups after treatment.The treatment group was better than that of the control group(P =0.001),and the excellent rate of the treatment group was higher than that of the control group.The efficacy of modified Macnab was evaluated at one month follow-up after treatment.The treatment group was superior to the control group(P<0.001),and the excellent rate of the treatment group was higher than that of the control group.(4)Both the peak temperature and the magnitude of temperature raised of the second measurement and the third measurement were higher than that of the first measurement maximum temperature(both P <0.05).Conclusion:(1)Traditional warm-needle therapy alleviates pain,lumbar symptoms and reduces low back pain in patients’ daily life both in short-term(after treatment)and long-term(1 month after treatment).These effects are better than ordinary acupuncture,and the effect is relatively stable,which worth clinical promotion.(2)During the warm needle process,the second and third procedures can achieve higher temperature and temperature rise than the first combustion.However,the third combustion can not achieve higher peak temperature and magnitude of temperature raised than the second combustion. |
