Objective:we retrospective analyzed the clinical data of patients with acute pancreatitis in pregnancy(APIP)in the First Affiliated Hospital of Nanchang University.Blood lipid,blood glucose,blood calcium,neutrophil absolute value and lymphocyte absolute value were analyzed to explore the independent risk factors of APIP;we discussed the diagnostic value of neutrophil to lymphocyte ratio(NLR)and blood glucose in APIP.Methods:We collected 61 patients with acute pancreatitis in pregnancy who had not been treated before treatment in the First Affiliated Hospital of Nanchang University from January 2014 to September 2019.The normal pregnant women with similar ages and gestational weeks in the same period were randomly included as the control group,collecting the general data and relevant biochemical test indexes of the two groups,Through statistical analysis,the triglycerides,cholesterol,high-density lipoprotein,low-density lipoprotein,blood glucose,blood calcium and NLR of the two groups were compared,and the possible risk factors of APIP were analyzed by multi factor Logistic regression analysis to explore the independent risk factors of APIP.The ROC curve was used to evaluate the diagnostic value of NLR and blood glucose in APIP.Results:APIP mostly occurred in the middle and late pregnancy,accounting for 95.08%.The main causes of APIP were hyperlipidemia(52.46%)and biliary tract disease(36.07%).Hyperlipidemia is more common with elevated triglycerides.Triglyceride,cholesterol and blood glucose of APIP patients were higher than those of normal pregnant women,while HDL,LDL and blood calcium was lower than that of normal pregnant women.There was statistical significance in the difference of triglyceride,cholesterol,HDL,LDL and blood glucose(P>0.05).There was no statistical significance in the difference of blood calcium.Compared with normal pregnant women,absolute neutrophils value and NLR in APIP patients increased,while absolute lymphocytes value decreased(P>0.05).ROC curve shows that both NLR and Glu have a certain extent diagnostic value for APIP,and have high sensitivity and specificity to APIP.NLR has a higher diagnostic value than Glu,with a cutoff value of 6.6172,a sensitivity of 90.2%,and a specificity of 94.3%.The cutoff value of blood glucose was 5.6650,the sensitivity was 77%,and the specificity was 76.2%.Conclusion:1.The increase of blood lipids or blood glucose during pregnancy can increase the incidence rate of APIP.APIP is often associated with abnormal triglycerides,cholesterol,blood glucose and blood calcium.High triglycerides is an independent risk factor for APIP,hyperglycemia may be an independent risk factor for APIP.2.NLR,as one of the inflammatory indexes,increases when APIP occurs and may have a certain extent diagnostic value in APIP.3.Blood glucose increases when APIP occurs,and it may have a certain extent value in the diagnosis of APIP.
