Objective:1.Objective to investigate the effect of cardiovascular risk factors on Angptl2 by detecting the level of angiopoietin like protein 2(Angptl2)in coronary artery serum under different risk factors.2.Objective to evaluate the role of serum angiopoietin like protein 2(Angptl2)in coronary heart disease classification and severity evaluation by detecting the distribution of coronary angiopoietin like protein(Angptl2)in different subtypes of coronary heart disease and subgroups.3.Objective to detect the level of angiopoietin like protein 2(Angptl2)in coronary artery before and after PCI treatment, and to analyze its efficacy in evaluating PCI in patients with coronary heart disease.Methods:1.A total of 156 patients with coronary angiography in were collected. The serum Angptl2 levels of the patients were measured and compared under the influence of different risk factors of cardiovascular disease. Objective to analyze the impact of cardiovascular risk factors on Angptl2.2.156 patients with coronary angiography in were collected, according to the clinical classification of the disease and severity of the results of the angiography group,of which: stable angina(SA)Group 24 cases, unstable angina(UA)Group 57 cases, acute myocardial infarction(AMI)Group of 43 cases,32 cases of negative control group were made.In UA,PCI was needed for 17 patients, and no PCI was required for 40 cases. In AMI,PCI was needed for 29 cases, and no PCI was required to treat subgroup 14 cases.3.To collect 156 cases of coronary angiography in patients with, including 124 cases of coronary heart disease [stable angina pectoris(SA)24 cases;unstable angina pectoris(UA)in 57 cases, including 17 cases receiving PCI treatment,4 patients received two PCI treatment;acute myocardial infarction(AMI)in 43 cases, including 29 cases receiving PCI treatment, received two doses of PCI treatment in 9 cases,32 cases of coronary arteriography negative control. The average interval between the second PCI operation and the first PCI operation was about a week. The changes of serum Angptl2 level of coronary artery before and after PCI,before and after the first and two PCI operation were detected respectively.Results:1.The analysis of clinical data showed that sex, smoking history, current smoking, and hypertension history had an effect on the level of serum Angptl2 in coronary artery(P <0.05).The current smoking situation and the history of hypertension are the most influential factors of Angptl2(current smoking p=0.006;the history of hypertension,p=0.042).2.The distribution characteristics of coronary artery serum ANGPTL2 in different disease types of coronary heart disease: The difference of ANGPTL2 level between the control group,SA group,UA Group and AMI Group was statistically significant(p<0.05);in comparison of each of the four groups, the P was less than 0.05,that is,the ANGPTL2 level of the control group was different from SA group,UA Group and AMI Group. The ANGPTL2 level of SA group was different from that in the control group,UA Group and AMI Group, and the ANGPTL2 level of UA Group was different from that in the control group,SA Group and AMI Group. The ANGPTL2 levels of AMI group were different from those of the control group,SA Group and UA Group. The coronary serum ANGPTL2 level in the control group was lower than that in the SA group, and the ANGPTL2 level of coronary artery in SA group was lower than that in UA Group, and the serum Angptl2 level of UA group was lower than that in AMI Group. In the AMI group,no PCI was required for the treatment of subgroup coronary artery serum ANGPTL2 level is lower than the need for PCI treatment subgroup, the difference is statistically significant(p<0.001);UA Group, between requiring PCI for the treatment ofsubgroup and no PCI for the treatment of subgroup coronary artery serum ANGPTL2 level difference was not statistically significant(p> 0.05).3.In AMI group, the changes of serum Angptl2 levels in coronary arteries before PCI,first and second PCI were statistically significant(p<0.05).The comparison between the two groups showed that, compared with pre-PCI subgroup, the changes of serum Angptl2 were statistically significant in the two subgroups after the first and second PCI(p=0.001);there was no significant difference in serum Angptl2 levels between the first PCI group and the second PCI group. In UA group, there was no statistical difference in serum Angptl2 levels among the subgroups of the UA group before, after the first PCI subgroup and after the second PCI subgroup,significance(p>0.05),so it cannot be considered that there is a difference in serum Angptl2 levels before PCI,after the first PCI,and after the second PCI on the UA.Conclusion:1.Through single factor analysis, sex, past smoking history, current smoking and hypertension history have influence on the level of Angptl2 in coronary blood. On the basis of single factor analysis, further regression analysis of multiple factors has been carried out to conclude that the current smoking situation and the history of hypertension have an influence on the level of coronary blood Angptl2.2.Among the different types of coronary heart disease, coronary serum Angptl2 levels were significantly different, which can play a role in the classification of coronary heart disease. The level of serum Angptl2 in coronary arteries has a certain evaluation effect on the severity of the disease in patients with AMI;however, for the evaluation of the severity of disease in UA patients,it needs to be comprehensively analyzed in combination with other risk factors of cardiovascular disease.3.PCI can be used as the preferred treatment for AMI patients, and the coronary serum ANGPTL2 level can be used as one of the biological indices for the evaluation of PCI in patients with AMI.The value of coronary artery serum ANGPTL2 alone in the evaluation of curative effect of PCI on the unstable angina patients is worth further study.
