Objective: By detecting the level of angiopoietin-like protein 3 in peripheral blood of patients with acute coronary syndrome,the level of plasma angiopoietin-like protein 3 in patients with acute coronary syndrome was observed,and the correlation between the level and the degree of coronary artery disease was discussed.Methods: A total of 136 patients underwent coronary art eriography examination in the Department of Cardiovascular Medicine of the Second Hospital of Shanxi Medical University from July 2019 to December 2019 were selected,including 90 patients in the ACS group(50 males,40 females)and 46 patients in the normal coronary angiography group(23 males,23 females).According to the results of the CAG group,48 patients with ACS group including unstable angina,42 patients with acute myocardial infarction,coronary stenosis level on the basis of Gensini score evaluation,divided into mild stenosis group(Gensini score 25 points or less,n = 32),moderate stenosis group(25 points < Gensini score 50 points or less,n = 30),severe stenosis group(> Gensini score 50 points,n = 28).The general clinical data and related biochemical indexes of the subjects were collected,and the level of Ang PTL3 was determined by enzyme-linked immunosorbent assay(ELISA).The differences in general clinical data,blood glucose,blood lipid and ANGPTL3 levels between ACS group and control group were analyzed and compared.Pearson correlation analysis was used to investigate the correlation between Ang PTL3 level and blood glucose,blood lipid and inflammatory indexes.One-way analysis of variance was used to compare the level of ANGPTL3 in each ACS subgroup to further explore the correlation between the level of ANGPTL3 and the degree of coronary artery disease.Results: 1.The body mass index,prevalence of diabetes,fasting blood glucose,total cholesterol,triglyceride,low density lipoprotein cholesterol,apolipoprotein B,lipoprotein A and hypersensitive C-reactive protein in the ACS group were all higher than those in the control group(P< 0.05),and the difference was statistically significant.Compared with the control group,age,sex,smoking history,drinking history,prevalence of hypertension,systolic blood pressure,diastolic blood pressure,high density lipoprotein cholesterol,apolipoprotein A1,P were > 0.05,the difference was not statistically significant.2.Compared with the control group,the level of serum Ang PTL3 in the ACS group was significantly higher(P < 0.001),and the difference was statistically significant.3.Pearson correlation analysis showed that serum Ang PTL3 level was correlated with FPG,TC,TG,LDL,Apo B and hs-CRP(r = 0.295,0.376,0.311,0.345,0.239,0.445,all P < 0.05).4.ACS group was divided into unstable angina pectoris group and acute myocardial infarction group.Multiple comparison(LSD method)showed that pair comparison within the group was P=0.043,< 0.05,both of which had significant significance.The level of angp TL3 in the control group was lower than that in the unstable angina pectoris group,and that in the unstable angina pectoris group was lower than that in the acute myocardial infarction group.Gensini score based on the ACS group were divided into mild stenosis,moderate stenosis,severe stenosis group,using the single factor analysis of variance,the results showed: ANGPTL3 level,mild stenosis < moderate stenosis group < severe stenosis group(F = 23.020,P < 0.001),LSD method are compared,and two mutual differences were statistically significant(P < 0.05).5.Pearson correlation analysis results showed that serum Ang PTL3 level was positively correlated with Gensini score.Conclusion: The level of serum ANGPTL3 in the ACS group was significantly increased,which was closely related to the metabolism of blood lipid,blood glucose and inflammatory response,and its level was positively correlated with the severity of ACS.In the future,the combined detection of ANGPTL3 could be used for early judgment and evaluation of the severity of disease and the level of coronary stenosis in patients with ACS,which has certain clinical application value.At the same time,targeted studies of ANGPTL3 may open up future therapeutic pathways for hyperlipidemia and cardiovascular diseases. |
