| Objective: As there were no criteria for evaluating the quality of transitional care of patients with coronary heart disease(CHD)after Percutaneous coronary intervention(PCI)around the world to construct a more suitable scale of continuing nursing evaluation index,and measured the validity and reliability of the indicators,which can be used to provide measurement tools for evaluating the effect of the continue nursing of patients with CHD after PCI,in order to lay a certain foundation for the standardized management of transitional care.Methods:(1)A Evaluation index was developed based on the Structural Framework of Self Management Theory(SMT)and Omaha effectiveness evaluation system,literature reviewing,clinic practice and semi-structured interviews for 12 relatives of patients with CHD after second PCI.(2)A total of 23 experts were selected by Delphi for three turns consultation.We calculated the questionnaire response rates,the authority coefficient and the harmony coefficient.According to screening criteria,the mean importance of index is beyond 3.5,the full score is beyond 20% and the coefficient of variance is less than 0.25 to select indicators.(3)Through 10 patients with second PCI to modify the evaluation index.(4)As certained the index weight and combination weight by AHP.(5)We investigated 231 cases with CHD after PCI with readmission by purposive sampling method.Then evaluated the reliability and validity of this index.Results:(1)we made an initial draft of transitional care quality evaluation index for patients with CHD after PCI,which included 4first-class indexes,21 second-class indexes,and 93 third-class indexes.(2)23 experts were enrolled in the investigation by Delphi.The rerates of the three-round consultation were 91.3%,81.0%,100%.The authority coefficient was 0.87?0.88?0.91.The harmony coefficients were 0.242?0.236?0.202,P<0.01.(3)After 3 turns experts consulations,the expert's coodination was gradually increased.Established the evaluation index of transitional care effectiveness of patients with CHD after PCI which included 4 first-class indexes,16 second-class indexes,and 46 third-class indexes.(4)Through investigation 10 patients,1 index were adjusted and 4indexes were deleted.(5)The weights and combination weights of theindexes were setted using AHP at the basis of the mean importance of index.The weights of the first-class indexes were 0.489?0.137?0.304?0.070.The ratio of CR index consistency at all levels are less than 0.10.(6)The questionnaire survey 231 cases,recycling of 228,recovery is 98.70%,effective recycling of 219,effective recovery is 94.81%.The Cronbach's alpha was 0.926 for the total index.The Cronbach's alpha of 4 first indicators were 0.600?0.768?0.918?0.798.The split half reliability was0.952.The index content validity index was 0.888.Factor analysis was used to extmct 6 factors,the cumulative variance contribution rate was 69.26%.Conclusion:We established the transitional care effectiveness evaluation index of patients with CHD after PCI.Which was systemis,scientific and widely,the weight of the index is reasonable and accuracy,and the index with good validity and reliability,could be used as a measure tool to provide reference for quality management of transitional care. |
