ObjectiveTo investigate the prevalence of sarcopenia in middle-aged and old people of Han,Tibetan and Qiang nationalities in Sichuan.To explore the distribution characteristics of sarcopenia in different ethnic groups,clarify the Correlation between sarcopenia and metabolism related factors,and provide reference for the prevention and intervention of sarcopenia.Materials and methodsA total of 4220 Han,Tibetan,and Qiang people aged 50 and above in Sichuan were randomly selected,and demographic characteristics and chronic diseases(including age,ethnicity,gender,marital status,cultural level,smoking,drinking,and chronic diseases)were collected using questionnaires.Comprehensive geriatric assessment was performed using the daily living ability scale(ADL,Activities of Daily Living)and geriatric depression scale(gds-15,geriatric depression scale-15).Physical fitness and body composition measurement(including height,weight,Walking Speed,grip strength,upper limb muscle strength,and human body composition)were conducted on site.The equipment used included tsinghua tongfang four-meter Normal Walking Speed Tester(cstf-5000 4-m Normal Walking Speed Tester),human body composition analyzer Inbody 770(Bio Space,Seoul,Korea),and grip strength Tester(EH101;Camry,zhongshan,China);Fasting venous blood was collected to test the metabolic indexes.Triglyceride(TG),Total cholesterol(TC),Low density lipoprotein(LDL),fasting blood Glucose(GLU0)and High density lipoprotein(High density lipoprotein)were detected by Japanese automatic biochemical analyzer(OLympus Au400).HDL-C),Albumin(ALB),Prealbumin(PA),Total protein(TP).Fasting insulin(Insulin0)was detected by a German automatic immune analyzer(Roche Cobase411),and 25-hydroxyvitamin D(Vitd)was detected by a U.S.automatic immune analyzer(Abbott i20005R).Limb muscle mass index(limb mass/height2,Appendicular lean mass/height2,ASMI),Grip strength(GS),and walking speed were used to diagnose sarcopenia.The subjects were divided into the sarcopenia group and the non-sarcopenia group,and then stratified the sarcopenia group and the non-sarcopenia group according to gender,BMI and age.Kolmogorov-smirnov method,t test,Wilcoxon rank-sum test,X2 test,binary Logistic regression analysis,one-way anova and other statistical methods were used to analyze the prevalence of sarcodynia in this population and its relationship with metabolic factors.Results1.The prevalence of sarcopenia in the total population of this study is 18.4%,of which the prevalence of males is 21.9%,the prevalence of females is 16.5%,the prevalence of males is significantly higher than that of females.The prevalence of sarcopenia in the Han nationality is 22.3%,the prevalence of Tibetan sarcopenia is18.2%,and the prevalence of Qiang sarcopenia is 11.8%.The prevalence of middle-aged and middle-aged people(50 ?59 years old)in the sarcopenia group was8.3%,and the prevalence of sarcopenia in the younger age group(60?69 years old)was17.1%;the prevalence of sarcopenia in the middle-aged and elderly people(≥70 years old)The rate is 41.6%.2.The levels of sarcopenia in the biochemical indexes HDL-C,TG,Vit D,Insulin0,PA and TP in this research laboratory were significantly lower than those in the non-sarcoidosis group.Visual impairment was significantly higher in the sarcopenia group(78.4%)than in the non-sarcopenia group(66.3%).3.According to the results of logistic multivariate regression analysis,the risk of sarcopenia increases with age,and old age is a risk factor for sarcopenia(OR=1.128,95%ci =1.106-1.115,P<0.001).Men were more likely than women to be risk factors for sarcopenia(OR=0.702,95% CI=0.500-0.985,P<0.001).The han nationality is a risk factor for sarcopenia compared with the qiang nationality(OR=0.452,95%CI=0.306-0.669,P<0.001).4.According to the results of logistic multivariate regression analysis,hypertension was a risk factor for sarcopenia(OR=2.128,95% CI=1.615-2.802,P < 0.001).Low body weight is a risk factor for sarcopenia(OR=3.363,95% CI=1.942-5.823,P < 0.001),and overweight is a protective factor for sarcopenia(OR=0.180,CI=0.036-0.084,P <0.001).The risk of sarcopenia in overweight people is 0.180 times that in normal people.Obesity was a protective factor for sarcopenia(OR=0.055,CI=0.036-0.084,P < 0.001).5.According to the results of logistic multivariate regression analysis,high level of PA was a protective factor for sarcopenia(OR=0.998,95% CI=0.996-0.999,P < 0.05).High levels of HDL-C were protective factors for sarcopenia(OR=0.738,95%CI=0.725-0.742,P <0.001).High levels of TG are risk factors for sarcopenia(OR=1.128,95% CI=1.027-1.277,0.036,P<0.05).High Insulin0 was a protective factor for sarcopenia(OR=0.897,95% CI=0.866-0.909,P < 0.001).High levels of Vit D were protective factors for sarcopenia(OR=0.972,95% CI=0.958-0.986,P < 0.001).In males,high levels of hdl-c,Vit D,Insulin0 and PA were protective factors for sarcopenia.In females,high levels of PA,Insulin0,and HDL-C are protective factors for sarcopenia,while high levels of TG are risk factors.Conclusion1.In this study,the prevalence rate of sarcopenia in middle-aged and elderly population was 18.4%,and the prevalence rate of males was higher than that of females.The prevalence rate of han nationality in the three ethnic groups was the highest,and that of qiang nationality was the lowest.The prevalence rate of sarcopenia was highest in the middle aged and lowest in the middle aged.2.Higher age,male,and hypertension are risk factors for sarcopenia.Compared with the qiang ethnic group,the han ethnic group is a risk factor for sarcopenia.3.Low body weight is a risk factor for sarcopenia,while overweight and obesity are protective factors for sarcopenia.4.Low levels of hdl-c,PA,Insulin0,vit D and high levels of TG are risk factors for sarcopenia.In males,low levels of HDL-C,PA,Insulin0 and vit D are risk factors for sarcopenia.In women: low levels of HDL-C,PA,Insulin0,and high levels of TG are risk factors for sarcopenia.
