| Objectives To investigate the relationship between Hp infection and NLR,PLR and lipid metabolism in peripheral blood of middle-aged and elderly T2 DM patients,and to understand the effects of Hp infection on inflammatory response and lipid metabolism in middle-aged and elderly T2 DM patients,so as to provide reference for the prevention and treatment of middle-aged and elderly T2 DM patients with Hp infection.Methods A total of 218 T2 DM patients who met the inclusion and exclusion criteria were divided into two groups: group A: negative for Hp infection(T2DM-HP_,n=91);Group B: Hp positive(T2DM-Hp+,n=127).Patients in group B were divided into group B1 according to the severity of Hp infection(mild,4.0≤DOB< 20,n=54).Group B2(moderate,20≤DOB < 40,n=38);B3 group(severe,DOB≥40,n=35).The general data,NLR,PLR,lipid levels and other data were compared between groups A and B and between groups B1,B2 and B3.The data of51 patients with successful Hp eradication before and after Hp eradication were recorded,which were divided into C1 group(before Hp eradication)and C2 group(after Hp eradication),respectively.The changes of PLR,NLR,blood lipid and other data of middle-aged and elderly T2 DM patients before and after Hp eradication were analyzed.SPSS25.0 software was used for statistical analysis.Results 1.There were differences in drinking history,NLR,ESR,CRP,TC,TG,HDL,LDL,APOB,PLIP and serum ammonia between T2DM-HP + group and T2DM-HP_group(P < 0.05).2.There were differences in sex,NLR,ESR,TC,TG,HDL,LDL,APOA-1,APOB,LPa,PLIP and serum ammonia among mild,moderate and severe T2DM-HP + groups(P < 0.05).3.There were differences in NLR,ESR,CRP,TC,TG,HDL,LDL,blood ammonia,blood urea and CYS-C before and after Hp eradication(P < 0.05).4.Logistic regression analysis: No drinking history and CRP < 3.13 mg/L were protective factors for Hp infection in middle-aged and elderly T2 DM patients.The effects of serum ammonia,NLR,TG,LDL and APOB on Hp infection in elderly T2 DM patients were statistically different(P < 0.05).5.NLR,TG,LDL,APOB,serum ammonia,drinking history and CRP had predictive value for T2 DM complicated with Hp infection,and AUC had statistical significance(P <0.05).Conclusions 1.Hp infection may aggravate the inflammatory response and lipid metabolism disorder in middle-aged and elderly T2 DM patients,and the inflammatory response and lipid metabolism disorder in middle-aged and elderly T2 DM patients may be reduced after the eradication of Hp.2.The levels of NLR,TG,LDL,APOB,CRP and drinking history were the influential factors of Hp infection in middle-aged and elderly T2 DM patients,and no drinking history and CRP < 3.13mg/L were the protective factors of Hp infection in middle-aged and elderly T2 DM patients.3.The levels of NLR,TG,LDL,APOB,CRP and drinking history have predictive value for Hp infection in middle-aged and elderly T2 DM patients. |
