Background: The global incidence of obesity is increasing annually and the pandemic tends to affect younger population.Childhood obesity has become a global public health problem,which not only increases social burden,but also leads to a variety of metabolic complications with short-term and long-term impacts on children’s health.Studies have shown that intestinal flora imbalance is related to obesity.Probiotics can improve inflammation by regulating intestinal flora composition,and in turn leading to weight loss.Yet further study is needed to assess the clinical effect of probiotics intervention on children with simple obesity.Objective: This study was designed to assess the effect of probiotic intervention using tablets with live Bifidobacterium and Lactobacillus on children with simple obesity.For this purpose,the changes of body mass index(BMI),glucose and lipid metabolism parameters and inflammatory factors before and after the intervention program were measured.Overall we set to generate evidence for developing efficient and feasible weight loss program and to evaluate the safety and effectiveness of probiotics in obese children as well.Methods: Fifty-four children diagnosed with simple obesity were enrolled at the Department of Endocrinology of Fuzhou Children’s Hospital of Fujian from February to September,2018.30 children and 24 children were randomly selected into probiotic and control groups.All individuals were subjected for dietary and physical intervention.The dietary intervention involved low fat and proper carbohydrate portions and calorie intake control: 800-1000kcal/d for 6-10 years old,1000-1200kcal/d for 10-14 years old.Physical intervention involved minimal 60 minutes aerobic exercise per day,minimal 5-day per week.In the probiotic group,the tablets with live Bifidobacterium and Lactobacillus were administered orally,3 times a day,4 tablets each time(0.5 g/tablet).Both groups underwent a 12-week intervention program.The height,weight and waist circumference were measured before and after the program,and the fasting venous blood was taken in the morning to detect blood lipids,blood sugar(FBG),insulin(INS),Triglyceride(TG),total cholesterol(TC),high-density lipoprotein cholesterol(HDL),low-density lipoprotein cholesterol(LDL),and calculated BMI,insulin resistance index(HOMA-IR).Tumor necrosis factor alpha(TNFα),serum interleukin-6(IL-6),and lipopolysaccharide binding protein(LBP)were detected by enzyme-linked immunosorbent assay(ELISA).Analysis was performed using SPSS 22.0 statistical software.Results: 1)30 children with simple obesity were in the probiotic group(19 boys,11 girls,mean age 10.02±1.93 years,mean BMI 25.54±0.21kg/m2),and 24 children were in the control group(15 boys,9 girls,mean age 9.61±1.98 years,BMI 25.21± 0.70 kg/m2).There were no significant differences in age and gender between the two groups.2)There were no significant differences in BMI,waist circumference,body fat,blood lipids,FBG,INS,HOMA-IR,TNFα,IL-6 and LBP before the intervention.After the intervention,the BMI,TG,FBG,INS,HOMA-IR were significant decreased compared with those before intervention(P < 0.05),and reduction was more pronounced in the probiotic group(P < 0.05).There was no significant difference in the levels of total cholesterol,low density lipoprotein and high density lipoprotein between the two groups after intervention(P>0.05);IL-6,TNFα,LBP levels decreased after intervention and more so for the probiotic group(P <0.05).Conclusion: Supplementation of tablets with live Bifidobacterium and Lactobacillus can significantly reduce BMI and body fat in children with simple obesity,improve glucose and lipid metabolism,as well as inflammatory status.The finding indicated that probiotic supplement can be used to treat children with simple obesity. |
