Study On The Difficulty Coefficient Of Operating Room Nursing Project

Objective:1.Identify operating room nursing projects that are appropriate for difficulty assessment and represent the workload of operating room nursing.2.To determine the difficulty dimension index and its weight for evaluating operating room nursing project.3.Determine the difficulty coefficient of operating room nursing project.Methods:Through the preliminary planning of the item pool of operating room nursing work by the method of literature data,the questionnaire survey was conducted in eight third-grade hospitals in guangzhou,and the expert meeting was held according to the questionnaire survey results to determine the operating room nursing work project and difficulty dimension index,and the weight of difficulty dimension index was determined by the analytic hierarchy process.Three rounds of letters were conducted to the operating room nursing experts by Delphi method,and the difficulty levels of each difficulty dimension index of the operating room nursing work project determined in this study were scored,and analyzed with EXCEL and SPSS 20.0 software.Result:1.Through the literature retrieval and in clinical departments to issue 126 questionnaires and 11 experts convened expert discussion meeting,ultimately determine the 78 operating room nursing work projects,and their weights are the difficulty of four dimensions,content and their weights are four,respectively is "skills complex degree(0.5463)","the operator requirements(0.2339)","operating load intensity(0.1085)" and"operation risk(0.1113).2.A total of 15 operating room nursing experts completed three rounds of Delphi consultation,with an average age of 46±5.33 years old,and the average operating room working time of 26.73±4.92 years,among which 46.7% were experts with deputy senior technical titles or above,all with bachelor's degree.The effective recovery rates of thethree rounds of questionnaires were 100%,88.2% and 100%.Three rounds of expert authority coefficient is 0.825,0.838,0.867.The coordination coefficients of the three rounds of Kendall were 0.535,0.680 and 0.776,and the P values of the three rounds of chi-square test were all less than 0.01.The expert score was of statistical significance.The Cronbach's a coefficient of three rounds of consultation questionnaires was 0.940,0.964 and 0.977.The top three of the difficulty coefficients of the operating room nursing work items were "Placement and observation of lateral traction bed(7.2687)","Skull fixation placement and observation(7.254)","Craniocerebral position and lateral decubitus position and observation(7.2433)".The last three were "Sterile single spread(2.046)","Replacement of intravenous fluids(1.9176)" and "Non-contact sterile gloves(1)".Conculsion:1.Consulting experts have high enthusiasm and authority to participate in the research,and they can obtain the difficulty score information with high reliability and value,making the difficulty coefficient statistical results objective and credible.2.78 operating room nursing work projects are all common clinical and suitable for difficulty assessment,including common basic nursing techniques,specialized coordination techniques,medical equipment nursing operation techniques,surgical position placement and observation,which can enhance the rationality of operating room nursing workload statistics.3.The difficulty dimension index covers a wide range of contents,and the weight value is reasonable,which reflects the connotation of operating room specialty and is applicable to the operating room specialty nursing field.4.The average value of difficulty coefficient of surgical position placement and observation is the highest,which is the key and difficult project reflecting the connotation of operating room specialty.The difficulty coefficient obtained in this study provides objective data support for the statistics of operating room nursing workload and the professional development of operating room nursing.
