Analysis On Clinical Efficacy Of Modified Danggui Shaoyao Powder Combined With Methylprednisolone In The Treatment Of Non-puerperal Mastitis Mass Stage With Liver Stagnation And Spleen Deficiency

Objective:To observe the clinical efficacy and safety of modified danggui shaoyao powder combined with methylprednisolone in the treatment of non-puerperal mastitis mass stage caused by liver stagnation and spleen deficiency.The changes of PRL,CD3,CD4,CD8,CD4/CD8 and NK cells were analyzed to provide a new therapeutic approach and clinical reference for the treatment of NPM mass stage.Methods:68 qualified cases were collected according to case inclusion criteria.Observation group and control group were randomly divided by random number table method with 34 cases in each group.Observation group: Including oral administration of western medicine(same as the control group),modified danggui shaoyao powder was added.Once per day both in the morning and evening,150ml/each,take warm decoction 30 minutes after the meal.Control group: Methylprednisolone 20 mg qd was administered orally in the morning.Doses were deduced from 16,12,8,and to 4 mg/d every 2 weeks.Both groups were treated for 10 weeks.Changes of tumor area,breast pain,local skin color,nipple discharge,TCM syndrome of liver stagnation and spleen deficiency,PRL,CD3,CD4,CD8,CD4/CD8,and NK cells were recorded before and after the treatment.Statistical analysis was performed at the end of all treatments and conclusions were made accordingly.Results:1.No statistical significance(P>0.05)was revealed in both groups in tumor area,breast pain,local skin color,nipple discharge,TCM syndrome,PRL,CD3,CD4,CD8,CD4/CD8,and NK cells before the treatment.All were demonstrated comparability.2.Clinical efficacy indicators:(1)Comparison within groups: Significant improvement was observed in both groups in mass area,breast pain and local skin color(P<0.01).In the observation group,satisfactory results were obtained in nipple discharge(P < 0.05),while no significant improvement was observed in the control group(P>0.05).(2)Comparison between groups: Improvement achieved in the observation group was superior to the control group in mass area,local skin color,breast pain and nipple discharge(P<0.05).3.TCM syndrome indicators: Scores of TCM syndromes in both groups exhibited a drop after treatment,and the difference was statistically significant(P<0.01).However,the treatment effect was more dominant in the observation group(P<0.01).4.Therapeutic evaluation:(1)Total clinical efficacy: After treatment,the total effective rate was 93.33% in the observation group and 80.00% in the control group,which demonstrated statistically significant differences(P<0.05).(2)TCM syndrome efficacy: After treatment,the total effective rate was 86.67% in the observation group and 53.33% in the control group,which demonstrated statistically significant differences(P<0.01).5.Laboratory indicators: PRL reduction in different degrees was observed in both groups after treatment.However,PRL level was significantly reduced in the observation group(P<0.01),while no marked decrease was found in the control group(P>0.05).Despite both groups presented effective regulation of CD3,CD4,CD8,CD4/CD8 and NK cells(P<0.01),the observation group developed superior in regulating PRL,CD3,CD8 and NK cell levels(P<0.01).The control group indicated better regulating ability of CD4(P<0.05),While comparing CD4/CD8,no significant difference was suggested between the two groups(P>0.05).6.Safety and adverse reaction evaluation:The incidence of adverse reactions was 0.00%in the observation group and 13.33% in the control group,respectively,displaying a statistical difference(P<0.05).Conclusions:1.Modified danggui shaoyao powder combined with methylprednisolone can obviously reduce the size of mass and significantly improve local skin color.The therapy also works well in relieving breast pain and inhibiting nipple discharge,which develops a better efficacy than western medicine.2.Compared with single administration of methylprednisolone,modified danggui shaoyao powder combined with methylprednisolone played a better role in improving TCM syndrome of liver stagnation and spleen deficiency,which significantly improved the clinical treatment effect and reduced the incidence of adverse reactions caused by western medicine.3.Modified danggui shaoyao powder combined with methylprednisolone can treat non-puerperal mastitis mass stage caused by liver stagnation and spleen deficiency by regulating PRL,CD3,CD4,CD8,CD4/CD8 and NK cells,which exhibited more effectively than single western medicine administration in an overall manner.
