Objective:To observe the clinical efficacy and safety of Regulating lungs and Reducing phlegm therapy in the treatment of Phlegm-Turbid obstructing of lung syndrome in elderly patients with acute exacerbation of chronic obstructivepulmonary disease.Methods:66 elderly patients,diagnosed with acute exacerbation of chronicobstructive pulmonary disease(Phlegm-Turbid obstructing of lung syndrome)from the geriatric and respiratory inpatient wards of the Affiliated Hospital of Chengdu University of Traditional Chinese Medicine,were divided into a treatment group and a control group of 33 cases each by the SPSS22.0 random number generator.Thecontrol group was treated with standard western medicine,while the treatment group with the method of Regulating lungs and Removing phlegm based on the standard western medicine treatment,with the addition and subtraction of Maxing Cangzhu Erchen Decoction as the specific Chinese medicine decoction.The treatment course of both groups was 10 days.By observing the changes between the two groups in the single points and total points of TCM syndromes,white blood cell count,neutrophil count,neutrophil percentage,C-reactive protein,procalcitonin,blood gas analysis,lung function,CAT score and m MRC score,before and after treatment,the clinical efficacy of the method of Regulating lungs and Removing phlegm can bevaluated.Simultaneously,blood routine,stool and urine routine,liver and kidney function,electrolytes,electrocardiogram and chest CT were checked and adverse reactions were recorded to evaluate the safety of the test.Result:A total of 3 cases were excluded and dropped off in this trial,and 32 cases in the treatment group and 31 cases in the control group were finally included.Theresults are as follows:(1)The total effective rate:The total effective rate of the treatment group in the treatment of elderly patients with acute exacerbation of chronic obstructivepulmonary disease(Phlegm-Turbid obstruction of lung syndrome)was 87.5%,while the the control group 77.42%,the treatment group was significantly better than the control group.(2)TCM individual symptoms and total scores of syndromes:The treatment group was significantly better than the control group in improving sputum volume,sputum color and sputum quality,and shortness of breath(P<0.01),and wheezing,chesttightness,appetite and abdominal distension were also better than the controlgroup(P≤0.05);after treatment,both groups can significantly improve the total scores of TCM syndromes,the treatment group is better than the control group(P<0.01).(3)Laboratory indicators:The treatment group improved NEUT、NEUT%、CRP、PCT and Pa O2 significantly better than the control group(P<0.01),also better inimproving WBC and FEV1%pred(P<0.05).(4)CAT、m MRC:The CAT score of the treatment group was significantly better than that of the control group(P<0.01),and the m MRC improvement was also better than the control group(P<0.05).(5)Safety:No adverse reactions occurred during the test,and the safety indicators did not show clinically significant deterioration.The safety evaluation was Grade 1.Conclusion:Regulating lung and Removing phlegm therapy can effectively improve the clinical symptoms and quality of life in elderly AECOPD patients withPhlegm-Turbid obstruction of lung syndrome.The clinical observation shows that this method has good safety and no adverse effects reactions.
