The Clinical Study Of BCG Combined With Gemcitabine In The Sequential Treatment Of High-Risk Non-Muscular-Invasive Bladder Cancer

Objectives To study the clinical effect of BCG combined with gemcitabine in the sequential treatment of high-risk non muscle invasive bladder cancer.Methods 132 patients with high-risk non muscle invasive bladder cancer were divided into group A(GEM group),group B(BCG group)and group C(BCG+GEM group)according to postoperative perfusion drugs and methods.There were 76 males and 56 females,the average age was 58 years(41-86 years),and the average follow-up was 17months(10-30 months).Through data collection,the recurrence rate,recurrence free survival time and adverse reaction rate of the three groups were studied.Results 1 The recurrence rate of gem group was 22.7%(10/44),BCG group was 15.9%(7/44),BCG+GEM group was 9.1%(4/44),BCG+GEM group was significantly lower than GEM group and BCG group(P<0.05).2 The recurrence free survival time of GEM group was(16.91±3.67)months;that of BCG group was(18.03±4.48)months;that of BCG+GEM group was(21.68±3.80)months;Kaplan Meier analysis showed that the cumulative recurrence rate of BCG+GEM group was better than that of GEM group and BCG group,but there was no statistical difference(P>0.05).3 The adverse reaction rate of GEM group was 13.6%(6/44),including 4 cases of bladder irritation,2 cases of gross hematuria;the adverse reaction rate of BCG group was 34.1%(15/44),2 cases of fever,12 cases of bladder irritation,2 cases of gross hematuria,and 1 case of rash;the adverse reaction rate of BCG+GEM group was 22.7%(10/44),2 cases of fever,6 cases of bladder irritation,and 2 cases of gross hematuria;the adverse events of three groups All of them improved by themselves or after treatment.There was significant difference among the three groups(P>0.05).The incidence of adverse reactions was the highest in BCG group.Conclusions BCG combined with GEM sequential treatment group,GEM and BCG treatment of high-risk NMIBC have curative effect.BCG combined with GEM sequential treatment is better than single drug treatment group,but there is no significant difference in the cumulative recurrence rate among the three groups.The incidence of adverse reactions in BCG group was significantly higher than that in BCG group.BCG combined with GEM sequential treatment group has high synergistic effect,definite curative effect,relatively small side effects,good tolerance and high acceptability.It can be recommended for postoperative perfusion treatment of high-risk NMIBC patients.Figure 1;Table 4;Reference 286...
