ObjectiveTo investigate the current level and influencing factors of compassion fatigue among emergency pediatric nurses in 7 hospitals of third-class A in Shandong Province,construct a cogitation reappraisal intervention plan,and evaluate the cogitation reappraisal intervention plan's effects of relieving compassion fatigue among emergency pediatric nurses.MethodsThis study is conducted in three stages.1.The first stage: the present situation investigation of compassion fatigue.To investigate the present situation of emergency pediatric nurses,and analyze the influencing factors of compassion fatigue.Taking 268 emergency pediatric nurses from 7 hospitals of third-class A in Shandong Province as the survey object,a self-made questionnaire of general conditions,professional quality of life scale for nurses and nursing work environment scale were used to conduct a question investigation.Using SPSS21.0 statistical software for data processing and analysis,Data were analyzed with t-test,one-way ANOVA,LSD test,multiple linear regression analysis to explore the influencing of the demographic variables,environmental factors and social support variables on nurse's Level of compassion fatigue.2.The second stage: the construction of cogitation reappraisal intervention program.According to the literature review and theoretical analysis,combined with the work characteristics and psychological characteristics of emergency pediatric nurses,a cogitation reappraisal intervention program for emergency pediatric nurses was constructed.Revise the plan after two rounds of expert consultations,the experts' opinions were basically consistent,and the consultation was completed to form the final cognitive reassessment intervention plan.3.The third stage: Evaluate the effect of cognitive reassessment intervention program.Cognitive reassessment(classroom instruction,situation simulation,case analysis,etc.)intervention methods were used to conduct cognitive reassessment intervention to emergency pediatric nurses in one hospitals,and quantitative and qualitative analysis were made to evaluate the effect of the intervention model on compassion fatigue.Finally,16 nurses were included in the control group and the experimental group.Cognitive reassessment intervention training was conducted for the experimental group,8 times,90 minutes each time;the control group was set as a blank control.A questionnaire survey of general information,compassion fatigue scale,Jefferson empathy scale and cognitive reassessment scale was done before and after intervention.Independent-samples t test and paired sample t test were conducted to perform a difference analysis comparison in compassion fatigue,Jefferson empathy and cognitive reassessment scores.Results1.Survey of compassion fatigue status and analysis of influencing factors:(1)Of the 268 emergency pediatric nurses,74(27.61%),130(48.51%),and 64(23.88%)were mild,moderate,and severe empathic fatigue.total score of compassion fatigue(111.35 ± 6.93),compassion satisfaction dimension(36.69 ± 7.67),job burnout dimension(23.29 ± 5.59),secondary trauma dimension(22.53 ± 6.46),total nursing work environment score is(90.90 ± 6.77)points.(2)Single factor analysis showed that the professional quality of life of emergency pediatric nurses is affected by personal factors such as different ages,educational backgrounds,nursing age,professional titles,monthly income,and number of night shifts.Emergency pediatric nurses are generally satisfied with the nursing work environment.The more satisfied with the nursing work environment,the higher the level of empathy satisfaction,the lower the level of job burnout and secondary trauma.(3)Multiple linear regression analysis shows that the predictors that have a significant impact on empathy satisfaction are participation in hospital affairs,sufficient manpower and materials,duties,and the ability of nursing managers;the predictors that have a significant impact on job burnout are high-quality nursing services and sufficient Human resources,education,monthly income,and medical care cooperation;the predictors that have a significant impact on secondary trauma are medical care cooperation,participation in hospital affairs,and educational background.2.Construction of cognitive reassessment intervention plan:Intervention program was formed after two rounds of expert consultation,including psychologists,nursing experts and head nurses guide and modify the framework and content of the cognitive reassessment plan.The plan includes awareness of emotions and cognitive reappraisal,challenge unreasonable beliefs,eliminate negative cognitive misleading,study of positive reassessment,study of optimistic reassessment,study of reassessment of rational beliefs,ABCDE,happy self,summary communication,and meet the future units.3.Evaluation of the effect of cognitive reassessment intervention program:(1)After intervention,the cognitive reassessment,perspective taking,emotional nursing care,and transposition thinking in the experimental group were significantly different from those before the intervention(P <0.01).(2)After the intervention,the compassion satisfaction,secondary trauma were significantly higher than before the intervention(P <0.05),and the improvement of occupational burnout was not obvious.Conclusion1.In this study,72.39% of emergency pediatric nurses were in moderate or severe empathic fatigue state,and were obviously affected by personal factors and their working environment.Nurses with poor nursing work environment and low job satisfaction are more prone to compassion fatigue.Nursing managers should pay special attention to such nurses and formulate corresponding preventive measures.2.Cognitive reassessment intervention is helpful to increase the level of empathy and satisfaction of emergency pediatric nurses,and improve their ability of perspective taking,emotional nursing care,and transposition thinking and self-reflection.3.Cognitive reassessment interventions can help emergency pediatric nurses improve mood,improve unreasonable cognition,and treat problems with optimism and rationality,but the effect of improving job burnout is not obvious.
