Part 1 Prevalence of hyperuricemia in community-based population with essential hypertensionBackground: The prevalence of hypertension and hyperuricemia is also an important risk factor for cardiovascular and cerebrovascular diseases. The prevalence of hyperuricemia in community essential hypertension population remains unclear. Objective: To investigate the prevalence of hyperuricemia in the population with essential hypertension. Methods: Using cluster sampling method to investigate the prevalence of hyperuricemia in the population with essential hypertension in Liaobu community,Dongguan City,Guangdong Province, using epidemiological questionnaire, physical examination and auxiliary examination to obtain the baseline data of the subjects, according to age groups:?45,45-55,55-65,65-75,and subgroup analysis of the baseline data to compare the prevalence of hyperuricemia between subgroups. Results: A total of 7334 cases were selected, the serum uric acid level of male is(426±102.48)?mol/L,the serum uric acid level of female is(366±94.42)?mol/L,the total prevalence was 50.3%,among which the prevalence rate of male was lower than that of female, but the difference was not statistically significant(50.2% vs 50.4%,P=0.903),the prevalence of men in each age group were 49.2%,45.9%,54.0%,52.3%,respectively.The prevalence for women were 34.8%,44.6%,54.1%,54.5%,respectively.The prevalence for men was higher than that for women before age 45(P<0.001),and the prevalence for men and women after age 45 was almost equal(P>0.05).Part 2 Risk factors associated with hyperuricemia in community-based patients with essential hypertensionBackground: There are various factors related to the incidence of hyperuricemia. At present, the related risk factors of hyperuricemia in factors are still unclear. Objective: To investigate and analyze the risk factors of hyperuricemia in community patients with essential hypertension. Methods: From January 2015 to December 2015,Liaobu community from Dongguan City,Guangdong Province was selected to complete the data of the resident essential hypertension population. The baseline data of subjects were obtained by epidemiological questionnaire, physical examination and auxiliary examination. The logistic regression model was used to analyze hyperuricemia related factors in male and female population respectively. Results: A total of 7334 cases were selected, and the multivariate Logistic regression model showed that waist circumference(OR=1.037),total cholesterol(OR=1.113)and triglyceride(OR=1.280)were risk factors for hyperuricemia in men.Body mass index(OR=1.035),waist circumference(OR=1.029),leukocytes(OR=1.104),alanine aminotransferase(OR=1.010),and triglycerides(OR=1.280)were risk factors for hyperuricemia in women. Conclusion: Waist circumference, total cholesterol, triglyceride may be the risk factors for hyperuricemia in men, height and body mass index, waist circumference, leukocytes, alanine aminotransferase, triglyceride may be the risk factors for hyperuricemia in women, and active control or intervention of these risk factors can prevent or slow the occurrence of hyperuricemia.Part 3 Trend of prevalence of hyperuricemia in community-based population with essential hypertensionBackground: The prevalence of hyperuricemia is on the rise all over the world, while there are few data related to the trend of hyperuricemia prevalence in patients with resident essential hypertension in the community. Objective: To analyze the trend of prevalence of hyperuricemia in community essential hypertension population. Methods: From January 2010 to December 2016,the resident population with essential hypertension was selected from Liaobu community in Dongguan City,Guangdong Province. The baseline data of subjects were obtained by epidemiological questionnaire, physical examination and auxiliary examination. The trend of serum uric acid level was analyzed by linear regression, and the trend of prevalence of hyperuricemia was analyzed by linear trend test. Results: The serum uric acid levels of men were 421 ± 102?mol/L,372 ± 100?mol/L,390 ± 106?mol/L,390 ± 107?mol/L,393 ± 103?mol/L,397 ± 103?mol/L,401 ± 119?mol/L,which shows general upward trend(P<0.001).The serum uric acid levels of women were 347 ± 92.3?mol/L,345 ± 93.8?mol/L,363 ± 95.9?mol/L,358 ± 96.4?mol/L,361 ± 92.8?mol/L,367 ± 94.4?mol/L,366 ± 110?mol/L,which shows general upward trend(P<0.001).From 2010 to 2016,the prevalence of hyperuricemia was 44.0%,41.2%,47.2%,46.7%,47.7%,50.3%,51.1% in the community resident hypertension population, which shows an overall upward trend(P=0.007).Among them, the prevalence of men was 9.8%,43.5%,48.8%,48.6%,48.9%,50.2%,52.7%,but there was no linear trend(P=0.148),the prevalence of women was 39.6%,39.4%,46.0%,45.0%,46.6%,50.4%,49.3%,with a linear trend(P=0.003).Conclusion: from 2010 to 2016,the serum uric acid level of the male and female population with essential hypertension in the community showed an overall upward trend, and the prevalence of hyperuricemia showed an overall upward trend, especially among which 65-75 age group in the male population and 45-55 years,55-65 years and 65-75 years in the female population.
