NLR,PLR And MPV Related To The Severity Of Obstructivesleep Apnea Syndrome

Objective To explore the value of neutrophils / lymphocyte(NLR),platelet / lymphocyte(PLR),and mean platelet volume(MPV)in obstructive sleep apnea syndrome(OSAS).Methods A retrospective case control study was used to collect the related laboratory data of84 patients with OSAS from January 2016 to March 2018 at the First Affiliated Hospital of Medical University Of Anhui,which were divided into three groups of light,medium and heavy according to the sleep apnea hypopnea index,and 26 patients were collected for the same period of simple snoring.The sleep parameters of patients were recorded with Polysomnography(PSG),And detection of peripheral blood neutrophils(WBC),neutrophils count(NC),lymphocyte count(LC),platelet count(PLT),platelet distribution width(PDW),platelet mean volume(MPV),NLR,PLR,total cholesterol(TC),triglyceride(TG),Low density lipoprotein cholesterol(LDL-C),BMI and night basal heart rate.Compare the differences between the above four indicators in four groups,and conduct a binary logistic multivariate regression analysis to explore the risk factors of the severity of OSAS patients,and perform a Pearson test to analyze the correlation between NLR,PLR,MPV and AHI,and evaluate with ROC curve The clinical application value of each index to OSAS patients.Results1.There was a significant linear correlation between NLR,PDW,MPV and AHI(P<0.05).It was further found that NLR,PDW,MPV and AHI all showed a positive correlation(r = 0.332,P = 0.002;r = 0.480,P < 0.001;r = 0.470,P <0.001),but there was no significant correlation between PLR and AHI(r =-0.166,P = 0.130).2.The area under the ROC curve predicted by NLR for OSAS is 0.836,the best cutoff value is 1.685,the sensitivity is 66%,the specificity is 92%,the positive predictive value is 96.5%,and the negative predictive value is 45.3%.The area under the ROC curve predicted by MPV for OSAS is 0.733,with the best cutoff value of 11.85,sensitivity of 51.2%,specificity of 96.2%,positive predictive value of 97.7%,and negative predictive value of 62.1%.3.There were statistical differences in BMI,TC,TG,LDL-C,average heart rate at night,NLR,PDW,MPV,minimum oxygen saturation,and average oxygen saturation among the groups(P <0.01).4.Patients between the ages of 40-60 years,higher BMI values,and faster night heart rate were independent risk factors for OSAS patients(P <0.05).Conclusions Peripheral blood NLR and MPV have important predictive value in judging the severity of OSAS patients,which can prompt clinicians to identify high-risk patients in a timely manner,take timely intervention measures,and further evaluate and identify cardiovascular risk factors.With the above biomarkers as guidance,it has important clinical auxiliary diagnostic value and good application prospect in the process of outpatient and clinical diagnosis and treatment.
