| Objective:To explore the differences of blood cell ratio neutrophil/lymphocyte ratio(NLR),platelet/lymphocyte ratio(PLR)and mean platelet volume/lymphocyte ratio(MPVLR)in patient with coronary heart disease(CHD)and coronary heart disease patient with obstructive sleep apnea hypopnea syndrome(OSAHS),and further analyze the level of NLR,PLR,MPVLR in patient with coronary heart disease and obstructive sleep apnea hypopnea syndrome,And their correlation with the degree of coronary artery disease.Methods:Research object:From September 2017 to January 2020,patients diagnosed with coronary heart disease by improved coronary angiography in the first affiliated hospital of kunming medical university were selected.According to the result of coronary angiography,patients were given a coronary Gensini score,and 102 patients with coronary heart disease who improved polysomnography(PSG)were eventually screened,according to the PSG results,the lowest SpO2,average SpO2 and the sleep apnea hypopnea index(AHI)of patients were counted.According to the recorded sleep apnea hypopnea index,the selected patients with coronary heart disease were divided into two groups:coronary heart disease combined with OSHAS group(AHI≥5 times/hour)a total of 65 cases and coronary heart disease group(AHI<5 times/hour)a total of 37 cases.Observation index:Record the neutrophil count,lymphocyte count,platelet count and mean platelet volume of each patient according to blood routine results,and calculate the NLR value,PLR value,MPVLR value.SPSS23.0 was used to analyze whether there were differences in NLR value,PLR value,MPVLR value and Gensini score between the two groups.According to the AHI value,the coronary heart disease combined with OSAHS group were divided into mild group(5 times/hour≤AHI ≤15 times/hour),moderate group(15 times/hour<AHI<30 times/hour),severe group>30 times/hour),and further analyze the differences in NLR value,PLR value,MPVLR value,and Gensini score between each subgroup,and to explore the correlation between NLR value,PLR value,MPVLR value and Gensini score and AHI.Results:(1)Compared with the coronary heart disease group and the coronary heart disease group,the differences in age,lymphocyte count,AHI,and the lowest Sp02 between the two groups were statistically significant(P<0.05).The gender,smoking,BMI,white blood cell count,neutrophil count,platelet count,mean platelet volume,total cholesterol,triglyceride,low-density lipoprotein cholesterol,high-density lipoprotein cholesterol,and average SpO2 were not statistically significant between the two groups of patients(P>0.05).(2)The difference between Gensini score,NLR and MPVLR in coronary heart disease combined with OSAHS group and coronary heart disease group were statistically significant(P<0.05),but there were no statistically significant difference in PLR(P>0.05).(3)The AHI and NLR levels of patient with coronary heart disease and OSAHS were positively correlated with Gensini score(respectively:r=0.33,P=0.01,r=0.35,P=0.01).(4)According to the AHI value,the coronary heart disease with OSAHS group was divided into mild group(27 cases),moderate group(18 cases)and severe group(20 cases).The differences in NLR,MPVLR and Gensini scores among the three groups were statistically significant(P<0.05),the differences in PLR were not statistically significant(P>0.05).Group two comparison between:① Comparison between severe group and mild group:NLR(P<0.01),MPVLR((P<0.01),Gensini score(P<0.01)differences were statistically significant;②Comparison between severe group and moderate group:Gensini score difference was statistically significant(P<0.05),while other indicators were not statistically significant(P>0.05).③There was no statistically significant difference between the moderate group and the mild group(P>0.05)(5)NLR,PLR,MPVLR and Gensini score were positively correlated with AHI in patient with coronary heart disease with OSAHS(Respectively:r=0.38,P=0.01,r=0.27,P=0.03,r=0.41,P=0.01;r=0.33,P=0.01).Conclusion:In patients with coronary heart disease and OSAHS,NLR,PLR,MPVLR are positively correlated with AHI;and AHI,NLR are positively correlated with the severity of coronary artery disease;Therefore,NLR,PLR and MPVLR may become auxiliary indicators for the diagnosis and evaluation of the severity of the patient with coronary heart disease and OSAHS. |
