Chronic heart failure is characterized by decreased heart function chronically and continuously. Cardiac resynchronization therapy(CRT) is also called atrial synchronous double ventricular pacing, which is non-pharmaceutical techniques. This techniques mainly implants three chamber pacemaker devices,stimulates left and right ventricle in synchronization way,improves the dyssynchrony of ventricle.This way can significantly improve the patients’cardiac function, reverse ventricular structure,improve the patients’athletic ability,improve the quality of life.Patients nursing who were undergoing CRT and the quality of life were especially important.The participants were asked to finish SF-36 and Minnesota heart failure quality of life Scale before and six months after CRT in this study. The quality of life, income, whether returning to work,and heart situation of the participants were collected. This study aims to explore the factors that effect the patients’quality of life, and provide the references for improving the quality of life of chronic heart failure after CRT.ObjectiveTo investigate the quality of life of the patients with Chronic heart failure after CRT, to explore the related factors that influenced patients’quality of life,to provide the theoretic basis for improving the patients’quality of life after CRT.MethodsSelf-control study was conducted in this study,120 patients performing CRT were asked to finish self-designed demographic data questionnaire, SF-36 and Minnesota heart failure quality of life Scale before and six months after CRT.The related factors were analyzed, then according to the investigation results, the targeted nursing interventions were taken.ResultsThough there were 120 participants selected at first, only 103 persons were involved in the follow-up interview(follow-up rate 85.83%), the results showed that:1. There were 65 males and 38 females, the age ranged from 45 to 71(55.85±7.43), After CRT,36 patients returned to their work,9 patients returned to work completely (8.7%),27 patients partly(26.2%),67 patients did not return to work.Among such 67 patients,30 patients retired (29.1%),3 cases were veteran patients (2.9%),29 patients were in sick leave(28.2%),5 patients had no work (4.9%). There were statistical differences on the scores of sf-36 between patients returning to work or not after CRT (P<0.05)2. There were statistical differences on the scores of sf-36 between patients returning to work or not after CRT (P<0.05),The patients’quality of life whose average monthly incomes were equal to or greater than 3000 yuan per person was higher than other three groups including below 1000 yuan, 1000-1999,2000-2999.3. Each dimension score of SF-36 and Minnesota heart failure quality of life Scale had improved obviously six months after CRT, and it was statistically significant (P<0.05)4. The patients’heart function improved significantly six months after CRT,72% patients’ heart function improved from the level of â…¢ã€â…£ to â… â…¡. Six-minute distances increased from 273 meters to 422 meters on foot. The distance of 6-minute walk test was positively correlated with sf-36 score.But MLHFQ score was negatively correlated with SF-36 score. (P<0.05)5.Univariate analysis and multiple stepwise regression analysis showed that, whether returning to work,personal income and cardiac function were the major factors that influencing the patients’ quality of life.Conclusion1.Patients’ heart function improved after CRT, and they can return to their work partly or completely.2.The patients’ quality of life has been significantly improved after CRT.3.Whether returning to work, personal income and heart situation are the major related factors that influencing the patients’ quality of life after CRT. |
