Objective: With the continuous development of society and the change of dietary structure brought by the improvement of living standards,the incidence rate of hyperuricemia has increased significantly in the whole country and even the whole world.In recent years,researchers found that hyperuricemia may be related to cardiovascular and cerebrovascular diseases.Patients with chronic renal failure usually have cardiovascular and cerebrovascular diseases,with high mortality.The purpose of this study is to explore the relationship between hyperuricemia and vascular disease in patients with chronic renal failure.Methods: 489 patients with chronic renal failure from July 1,2014 to July 1,2019 in the Affiliated Hospital of Qingdao University were selected for retrospective study,including 295 males and 194 postmenopausal females.The researchers collected the general data of patients’ sex,age,height,weight,blood pressure and previous history,and collected fasting biochemical indexes such as blood uric acid value,blood calcium and triglyceride.The calcification and atherosclerosis of coronary artery were evaluated by chest CT and vascular ultrasound.According to the above data,489 patients with chronic renal failure were divided into hyperuricemic group and normal uricemic group,252 of which were hyperuricemic group(the level of uricemic acid was 560.75±90.17 ummol / L),237 of which were normal uricemic group(the level of uricemic acid was 313.41±60.65 ummol / L).Using the method of medical statistics,the continuous variables were described by means of mean ± standard deviation,and the qualitative data were expressed as rate.Using SPSS statistical software to analyze the correlation between hyperuricemia and general indicators such as gender,age,height,weight,etc.,and to analyze the correlation between hyperuricemia and biochemical indexes such as serum creatinine,blood lipid,blood calcium and coronary artery calcification,atherosclerosis and other vascular diseases.Results:(1)The body mass index,serum creatinine and serum phosphorus levels of hyperuricemia group were significantly higher than those of normal uric acid group(P < 0.05).The serum calcium and estimated glomerular filtration rate(e GFR)of hyperuricemia group were lower(P < 0.05).There was no significant difference in triglyceride and total cholesterol levels between the two groups(P > 0.05).(2)The serum uric acid level of 489 patients with chronic renal failure was positively correlated with systolic blood pressure,diastolic blood pressure,serum creatinine,body mass index(BMI),blood phosphorus,calcium phosphorus product(r=0.16~0.40,P<0.05),negatively correlated with age,e GFR and blood calcium(r=-0.14,-0.19,-0.17,P<0.05),and not significantly correlated with total cholesterol and triglyceride(P > 0.05).(3)Taking blood uric acid as independent variable and systolic pressure,diastolic pressure,blood creatinine,BMI,blood phosphorus,blood calcium,e GFR,calcium phosphorus product as dependent variable,linear regression analysis was carried out.The results showed that with the increase of blood uric acid,systolic pressure,diastolic pressure,blood creatinine,blood phosphorus,BMI,calcium phosphorus product also increased gradually.With the increase of serum uric acid,e GFR and blood calcium decreased gradually.(4)The patients in hyperuricemia group were more likely to suffer from coronary calcification,atherosclerosis,hypertension and craniocerebral lesions(P < 0.05).The influence of hyperuricemia on vascular diseases was explored by logistic regression analysis,which confirmed the above viewpoints,including the incidence of coronary calcification(OR=2.895,P<0.05),the incidence of atherosclerosis(OR=3.476,P<0.05),the incidence of hypertension(OR=2.935,P<0.05),the incidence of craniocerebral diseases(OR=6.340,P<0.05),cardiac enlargement(OR=2.092,P<0.05)were all increased.Conclusion: In this study,aiming at patients with chronic renal failure,a special group,we studied the influence of high uric acid on blood vessels,and found that high uric acid has significant correlation with cardiovascular and cerebrovascular diseases such as hypertension,coronary calcification,atherosclerosis,cardiac enlargement and craniocerebral diseases.This is of great significance to the management of patients with chronic renal failure in clinical work.Early intervention of high uric acid status in patients with chronic renal failure may reduce the incidence of cardiovascular and cerebrovascular diseases and improve the prognosis of patients. |
