Analysis Of Anxiety,Depression,Insomnia And Their Influence On Quality Of Life And Related Factors In Patients With Malignant Tumors

Objective: With the transformation of the "bio-psychological-social medical model",malignant tumor has become a physical and mental disease.In addition to the pain brought by the disease itself,malignant tumor often leads to the mental disorder and sleep disorder of patients,resulting in the decline of the quality of life.Mental disorders are mainly composed of anxiety,depression;sleep disorder is mainly composed of insomnia.In this study,anxiety,depression,insomnia and quality of life of patients were investigated through questionnaires,to explore the related factors leading to anxiety,depression and insomnia in tumor patients,to explore the impact of anxiety,depression and insomnia on quality of life,and to explore the correlation between anxiety,depression and characteristics of different sleep disorders.Methods: A total of 190 inpatients from the fourth department of oncology,Sheng Jing hospital affiliated to China Medical University were collected,and the cases were collected from January 2018 to September 2019.The general demographic characteristics of the patients were collected,and the Hamilton Anxiety Scale(HAMA),Hamilton Depression Scale(HAMD),Athens Insomnia Scale(AIS),Quality Of Life(QOL)were performed.SPSS 25.0 software was used to describe the demographic characteristics of patients by frequency.Nine factors,such as gender,age,marital status and income,were selected to analyze the relationship between these factors and depression,anxiety and insomnia by single factor analysis of variance.Logistic regression was used to analyze the risk factors of anxiety,depression,insomnia and quality of life.Spearman correlation analysis was used to analyze the correlation of anxiety,depression,insomnia and quality of life and to analyze the correlation between anxiety,depression and different characteristics of sleep disorders.The test level was P<0.05 as a significant difference,P > 0.05 as no statistical significance.Results: Among the 190 hospitalized patients with cancer,the detection rate of anxiety was 61.6%(117 cases),the detection rate of depression was 53.6%(102 cases),and the detection rate of insomnia was 54.7%(104 cases).Spearman correlation was used to analyze the positive correlation between anxiety,depression and insomnia scores,whilethe negative correlation was found between anxiety,depression and insomnia scores and quality of life scores.Gender,age,marital status,income and other factors were selected for one-way ANOVA.Gender showed significant differences in anxiety,depression and insomnia in tumor patients(P<0.05).Types of medical insurance showed significant difference in depression in tumor patients(P<0.05).Occupation and treatment showed significant differences in insomnia(P<0.05)in tumor patients.Multivariate Logistic regression analysis showed that gender was an independent risk factor for anxiety,depression and insomnia.Depression was less likely to be reported in patients with off-site health insurance.Patients who have work were less likely to report insomnia,and patients in advanced supportive care were more likely to report insomnia.Occupational status,income satisfaction and treatment had an impact on the quality of life of tumor patients(P<0.05).Spearman correlation analysis showed difficulty in falling asleep,difficulty in maintaining sleep and waking up early were positively correlated with anxiety level of tumor patients,and waking up early was positively correlated with depression level of tumor patients.Conclusion: 1.The incidence rates of anxiety,depression and insomnia in tumor patients were high(61.6%,53.6% and 54.7%,respectively).2.Anxiety,depression and insomnia were positively correlated in tumor patients and the higher the scores of anxiety,depression and insomnia in tumor patients,the worse the quality of life of patients.3.Gender has a significant effect on anxiety,depression and insomnia in tumor patients and was an independent risk factor.Patients with off-site health insurance were less likely to report depression.Patients who have work were less likely to report insomnia,and patients in advanced supportive care were more likely to report insomnia.4.Occupational status,income satisfaction and treatment affected the quality of life of tumor patients.5.Difficulty in falling asleep,difficulty in maintaining sleep and waking up early were positively correlated with anxiety level of tumor patients,and waking up early was positively correlated with depression level of tumor patients.
