Research BackgroundThe whole world is facing the same problem-"ageing population".According to statistics,the population of 60 years and over in mainland China accounts for 18.1%of the total population,of which more than 180 million old people suffer from chronic diseases and suffer from one or more types.The proportion of sick elderly is as high as 75%,and the number of disabled and semi-disabled elderly is about 40 million.Shandong Province,as the country's most populous province,has entered a period of rapid population aging.Among them,Jinan City is one of the cities in Shandong Province and even the country that entered the population aging earlier.Faced with such difficulties,the traditional old-age care model has been impacted,and the elderly and the country need a new force to effectively address the needs of old-age care.China's own medical and health service resources are limited,and are independent of the old-age service system.The service supply efficiency is low,which cannot meet the effective needs of the elderly.The problem of healthy old-age care needs to be effectively resolved.The state has issued a series of documents for this purpose.The elderly nursing staff is the practitioner and mainstay of various policies and programs.Only by building a team of senior nursing staff and a professional and high-quality service team can each policy be put into practice,and can the elderly cause be pushed to a new level of "healthy senior care".Research purposesIn order to actively face the problem of population aging in our country,overcome the "pain points" of middle-aged and elderly people who need long-term care in the "integration of medical and nursing care",and strive to promote the new social trend of "active old-age care and sunshine old-age care".—The elderly care workers start by analyzing the status of their work and training and the factors that affect the training needs,and find and propose solutions to the problems of limited skills of care workers,put forward suggestions for improving the training system and strengthening the construction of institutions,and strive to promote the development of social welfare.Research methodsThe research materials mainly come from relevant literature,policy documents,questionnaire data and interview data from the field survey.The questionnaire survey targets were randomly selected senior care workers in the sample institutions and medical staff working in the sample institutions with medical conditions.There were 246 nurses;a total of 68 medical staff.By investigating and collecting the different characteristics,work status,training situation,and subjective colors of difficult-to-quantify data of elderly care workers,we found problems at this stage,analyzed the factors affecting their training needs,and gave corresponding countermeasures and suggestions.Based on the data obtained from the survey,using descriptive analysis,single factor analysis,multiple linear regression to analyze the factors that correspond to the training needs of elderly care workers,supplemented by interview information,looking for effective ways to improve the skill level of the elderly care workers.Data collation and analysis are mainly done through Epidata 3.1 and SPSS 19.0.Research results(1)The elderly care workers are mainly female,accounting for 80.41%of the 246 elderly care workers interviewed.The youngest caregiver interviewed was 20 years old,the longest was 69 years old,the average age was 44.25 years old,and the number of middle-aged and elderly people aged 46 to 69 years was the largest,accounting for 71.8%.In addition,most of the interviewees had rural household registration;the medical staff in pension institutions were mainly female,accounting for 80.28%;more people had urban household registration,accounting for 56.72%.(2)The knowledge structure and professional structure of nursing care workers:the surveyed nursing staff mainly concentrated in high school or vocational high level,accounting for 41.15%of the total number,and nearly nine adults have no relevant professional learning experience;it shows that most nursing staff are still in the middle With a low level of knowledge,there is a shortage of professional nursing staff in Jinan pension institutions,and they need more nursing experience.Among the 68 medical staff interviewed,there are too many nurses,accounting for 56.92%,which is related to the nature of the work of elderly care in pension institutions.Nurses have more "places to use" in the institution.All are licensed to work,and have the qualifications to practice.(3)Current status of training for senior care workers:Among the 246 interviewees,146 failed to hold the qualification certificates and other practice certificates issued by the state;half of them did not participate in pre-job training,and The training content is mainly at the level of professional ethics and basic nursing skills;the content of job training is old-fashioned and the knowledge is narrow.After entering the job,training is still required.78.86%of the elderly caregivers continue to receive post-employment training,the training rate has increased significantly,but the training content is still a similar part of pre-job training.Nursing staffs working abilities are inadequate and cannot meet the needs of society and institutions.Training must take a formal and deep road.(4)The current status of the work of elderly care workers:most of the interviewed elderly care workers engaged in the industry within 3 years,accounting for 74.89%of the total number;the longest reached 15 years,and the working age was 10 to 15 years There are fewer senior caregivers among them,accounting for only 5.29%of the total number.More than half of the nursing staff interviewed did not sign a contract,and among those who signed the contract,the contract period was mainly 12 months,accounting for 72.90%of the contract signings,and the longest contract period was 60 months,reflected from the side It shows the characteristics of high mobility of the industry.According to the survey,the working hours of nursing care workers are long and continuous.9 to 16 hours is the most common working time of nursing care workers per shift in the survey,accounting for 61.97%of the total number.It has the characteristics of high intensity and sustainability.The medical staff interviewed usually enter the nursing home through professional employment and signing labor contracts.This reveals that qualified pension institutions can employ medical personnel through more extensive publicity and recruitment methods to solve the medical needs within the institution.Medical staff working in old-age care institutions have roughly the same content as grass-roots medical institutions such as community health service centers,and mainly provide simple basic medical services.(5)Analysis of the influencing factors of the training needs of elderly care workers in Jinan City After statistical analysis,six factors including the health status of nursing staff,the length of service in the job,the length of service in the institution,job satisfaction,job preference,and satisfaction with their own service ability have the closest influence on the training needs of nursing staff in pension institutions factor.Conclusion and suggestionAccording to research,the level of job satisfaction of senior nursing staff in pension institutions in Jinan is high,and the level of nursing skills meets the standards.However,the continuous working hours and heavy workload,the low quality of employees still exists;the training rate affer entry is significantly higher than before employment Training,and training demand increased.This study found that the close factors that affect the training needs of pension care workers are not age,household registration,gender and other objective factors.In the end,six subjective factors are obtained,which also proves the subjective initiative reflected by the training needs.Based on the results of this study,the following suggestions are made:(1)relax the entry conditions and improve the entry barriers for the skills of pension care workers;(2)encourage conditional pension institutions to actively open medical departments and recruit medical staff;(3)pension institutions Institutional construction should be comprehensively planned,and cultural construction should be emphasized to create its own unique culture;(4)The government should find out the qualifications and facts of pension institutions,ban or merge under-qualified pension institutions,and vigorously support institutions with policies and funds.
