| Objective(s):Using the epidemiological sampling survey method to investigate the quality of adult male semen in Yunnan for the first time,and discuss its related important influencing factors,provide feasibility suggestions for the prevention and treatment of infertility,accurately guide adult male eugenics and postnatal care,maintain a reasonable and healthy birth rate,promote The healthy development of Yunnan population strategy.Methods:First,in this study,the stratified multi-stage random cluster sampling method was used to select communities in Wuhua District and Panlong District,Kunming City,and villages in Mengzi City and Luchun County,Honghe Prefecture,Yunnan Province.Calculate the sample size of the flow test,determine the inclusion and exclusion criteria,and recruit 436 qualified volunteers as the research object.Secondly,the research subjects independently filled out the uniformly-developed"Yunnan Province Adult Male Fertility Status Questionnaire".The questionnaire involves five items:general information,living environment,living habits,post-marital fertility information,and sexual life satisfaction;After 3-5 days of abstinence,semen was collected by masturbation.The researchers analyzed the semen with the LensHooke TM X1 PRO(X1 PRO)semen analyzer to obtain semen parameters.Finally,the semen quality was evaluated according to the fifth edition of the WHO semen analysis standard,and the semen examination parameters and questionnaire data were counted.The SPSS 20.0 statistical software was used to perform t-test and chi-square test on the data.The forward stepwise regression method was used to analyze the single factor.The statistically significant independent variables in the results were analyzed by multivariate logistic regression analysis to explore the important related factors that affect the quality of adult male semen in Yunnan,and the receiver operating characteristic curve(ROC)was used to predict the effectiveness of related semen abnormalities make an evaluationResults:1.Results of adult male semen quality survey in Yunnan area:Of the 436 respondents,52.29%had abnormal semen,of which the sperm activity rate(PR+NP)abnormality accounted for 59.21%,and the forward motor sperm activity rate(PR)abnormality 45.18%,abnormal sperm morphology accounted for 42.98%,and abnormal sperm concentration accounted for 39.91%.2.Single factor logistic regression analysis of adult male semen quality in Yunnan:age,body mass index,region,ethnicity,occupational exposure,smoking,drinking,type of underwear,diet preferences,psychological stress score,air quality score,drinking water quality score and sex There was a difference between the life satisfaction score and the adult male semen quality in Yunnan(P<0.001).3.Regional differences in the quality of adult male semen in Yunnan:Among the 228 persons with abnormal semen,communities in Wuhua District of Kunming City accounted for 42.98%,communities in Panlong District of Kunming City accounted for 38.16%,and villages in Mengzi City of Honghe Prefecture accounted for 12.28%.Villages in Luchun County of Honghe Prefecture accounted for 6.58%.The difference in sperm activity rate(PR+NP)in different regions was statistically significant(P<0.05).The sperm activity rate(PR+NP)was highest in adult males in Luchun County,Honghe Prefecture,at 38.40%.4.Ethnic differences in the quality of adult male semen in Yunnan:Among the 228 semen abnormalities,the Han nationality accounts for 75.00%,the Yi nationality accounts for 10.09%,and the Hani nationality accounts for 6.14%.The difference in total sperm count among different ethnic groups was statistically significant(P<0.05).The total sperm count was the highest among adult males of Han nationality at 65.72×106/ml.5.Differences in semen quality and lifestyle habits among adult males in Yunnan:sperm activity rate(PR+NP),forward motor sperm activity rate(PR)and total sperm count of smokers were significantly lower than those of non-smokers(P<0.05).The total sperm count of drinkers was significantly lower than that of non-drinkers(P<0.05).The difference of the forward exercise sperm activity rate(PR)between different dietary preference groups was statistically significant(P<0.05).The difference of forward motor sperm activity rate(PR)between different types of underwear was statistically significant(P<0.05)6.Adult men’s semen quality and personal differences in Yunnan area:the sperm activity rate(PR+NP),forward movement sperm activity rate(PR),and total sperm count in different age groups were statistically significant(P<0.05).There was a statistically significant difference in semen volume among people exposed to different occupations(P<0.05).The difference in total sperm count among different BMI groups was statistically significant(P<0.05).7.Multivariate logistic regression analysis of adult male semen quality in Yunnan:age,body mass index,region,smoking,alcohol consumption,type of underwear,dietary preferences,psychological pressure,and sexual life satisfaction are independent factors that affect the quality of adult male semen in Yunnan(P<0.05).The top four important influencing factors are psychological pressure,sexual life satisfaction,region,and drinking.8.Receiver work characteristic(ROC)curve of psychological stress score and sexual life satisfaction score:the maximum area under the curve of psychological stress score is 0.896,the area under the curve of sexual life satisfaction score is 0.836,psychological stress score and sexual life Satisfaction scores predict that abnormal semen quality is effective.Conclusion(s):1.The semen quality of adult males in Yunnan region has declined,and the abnormal semen quality is mainly due to abnormal sperm activity rate(PR+NP)2.Important factors related to semen quality of adult males in Yunnan:age,occupation,smoking,drinking,dietary preferences,body mass index,region,ethnicity,type of underwear,psychological pressure,air quality,drinking water quality,sexual life satisfaction;Among them,age,body mass index,smoking,drinking,and psychological stress are risk factors,while region,type of underwear,diet preferences,and sexual life satisfaction are protective factors.3.Adult men with higher age after age 35,more smoking,more alcohol,and overweight people(body mass index≥24)with a higher body mass index are more likely to have abnormal semen;Lvchun village areas,habits Adult men wearing casual loose underwear and eating fruits and vegetables prefer less chance of abnormal semen quality4.Psychological stress score predicts abnormal sensitivity and accuracy of adult male semen in Yunnan area,sexual life satisfaction score predicts high specificity of adult male semen abnormality in Yunnan area,critical value of psychological pressure score is 34.5 points,sexual life satisfaction score The critical value is 40 points.5.There is a shortage of grassroots male doctors.Grassroots hospitals,as the first stop for grassroots people to obtain medical services,should pay attention to male reproductive health education and strengthen discipline construction. |
