Objective To investigate the relationship between serum uric acid(SUA)and carotid intima-media thickness(CIMT)in Dalian.Methods Based on the REACTION study initiated by the Chinese Medical Association,Dalian community residents aged ≥ 40 years old who participated in the survey from August 2011 to December 2011 were selected to collect their gender,age,basic medical history,medication history and other relevant information,and their CIMT level was detected.Taking CIMT < 1.0mm,i.e.CIMT was not thickened as the inclusion condition,taking the existing history of stroke,coronary heart disease or other heart diseases as the exclusion condition;using drugs affecting uric acid metabolism in the past two weeks;and taking incomplete data as the exclusion condition.The height,weight,body mass index(BMI),systolic blood pressure(SBP),diastolic blood pressure(DBP),SUA and high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),total cholesterol(TC),Triglycerides(TG),fasting plasma glucose(FPG),2 hours plasma glucose(2h PG),glycated hemoglobin(Hb A1c)were measured.After 3 years,CIMT was tested again for the selected subjects.1,574 subjects(394 males and 1,180 females)with complete data were selected for analysis.According to the baseline SUA quartile level,the subjects were divided into four groups: Q1,Q2,Q3 and Q4;M1,M2,M3 and M4;F1,F2,F3 and F4.To compare the difference of CIMT thickening incidence after 3 years in each group,and to analyze the relationship between baseline SUA level and CIMT thickening after 3 years.Results 1.In this study,1,574 cases were enrolled.After 3 years,there were 1,374 cases with CIMT thickening.The incidence of CIMT thickening was 87.3%.The incidence of CIMT thickening in Q1,Q2,Q3 and Q4 was 79.8%、86.1%、90.9% and 92.6% respectively.Except for Q3 and Q4 group,there were significant differences among the other groups(chi square of Q1 and Q2,Q3 and Q4 group was χ2 = 0.639,P = 0.018,χ2 = 0.397,P = 0.000,χ2 = 0.315,P = 0.000;chi square of Q2 and Q3 and Q4 was χ2 = 0.621,P = 0.036,χ2 = 0.492,P = 0.003).Among the 1,180 female subjects,999 had CIMT thickening three years later.The incidence of CIMT thickening was 84.7%.The incidence of CIMT thickening in F1,F2,F3 and F4 was 77.3%,84.1%,86.2% and 91.4% respectively,Except for F2 and F3,there were significant differences among the other groups(chi square of F1 and F2,F3 and F4 were χ2 = 0.645,P = 0.035,χ2 = 0.544,P = 0.005,χ2 = 0.320,P = 0.000;chi square of F2 and F4 was χ2 = 0.497,P = 0.007;chi square of F3 and F4 was χ2 = 0.590,P = 0.048).In 394 male subjects,there were 375 cases with CIMT thickening after 3 years.The incidence of CIMT thickening was 95.2%.The incidence of CIMT thickening was 98.0%,97.9%,92.9% and 91.8% in M1,M2,m3 and M4 groups,respectively.There was no significant difference in the incidence of cumulative thickening between groups(χ2 = 6.820,P = 0.078),but the incidence of CIMT thickening decreased with the increase of SUA level.2.The overall,female and male subjects all took CIMT thickening after 3 years as the dependent variable,and gender(for the overall),age,height,weight,BMI,SBP,DBP,SUA,HDL-C,LDL-C,TC,TG,FPG,2h PG,Hb A1 c as the independent variable to carry out binary logistic regression analysis.The method of backward: LR was used to explore the influence of CIMT after 3 years in each population.The results showed that SUA and age were the factors influencing the thickening of CIMT in male subjects.After adjusting for age,the risk of CIMT thickening decreased with the increase of SUA level(OR = 0.992,95% CI: 0.985-0.999,P < 0.05);gender,age,BMI,HDL-C,TG and Hb A1 c are the factors that influence CIMT thickening in the overall study population,and SUA does not enter the equation;age,BMI,HDL-C,TG and Hb A1 c are the factors that affect CIMT thickening in the female study population,and SUA also does not enter the regression equation,suggesting that there is no correlation between SUA level and CIMT thickening in the overall and female population sex.Conclusions 1.For male:the incidence of CIMT thickening decreased with the increase of SUA level,which was the protective factor of CIMT thickening;2.There was no correlation between SUA and CIMT thickening in overall and female.
