Objective To explore the safety and efficacy of subfascial endoscopic perforator surgery(SEPS)combined with laparoscopic light source to guide injection of Aethoxysklerol in the treatment of venous ulcers of lower limbs.Methods Regression analysis was performed on 110 patients with venous leg ulcers who underwent surgical treatment in our hospital from January 2013 to March 2019.According to whether or not to use Aethoxysklerol during surgery,they were divided into group A: SEPS+endoscopic light source guided by injection beam and sclerotherapy;group B: only SEPS.There were61 cases in group A and 49 cases in group B.The two groups were compared between the two groups.The operation time,intraoperative blood loss,ulcer healing time,hospitalization time,treatment cost and other indicators were observed.The postoperative complications of the two groups were observed:subcutaneous hematoma,cough,dizziness,wound infection,fever,Skin necrosis,DVT(deep vein thrombosis),postoperative ulcer healing time,and VCSS scores were used to assess the improvement in lower limb symptoms at6 months after surgery.Results In group A,the operation time,a number of surgical incisions,intraoperative blood loss,postoperative hemoglobin,postoperative pain on day 1 and day 3,length of hospital stay,hospitalization cost,wound infection,and ulcer healing time were better than group B(P<0.05);and the difference between the two groups in postoperative platelet count,white blood cell count,D-dimer value,postoperative pain,dizziness,hematoma,fever,skin necrosis and DVT Academic significance(P>0.05).Conclusion1)Compared with SEPS alone,SEPE combined with endoscopic light source injection of dodocanol for the treatment of venous ulcers in the lower extremities is definitely effective.It has the advantages of less trauma,fast recovery,low cost,and shorter hospital stay.Further research and follow-up are needed.2)Injection Aethoxysklerol treatment is simpler to operate,does not require large equipment,avoids the use of ultrasound during surgery,and more intuitively observes the release of a subcutaneous vein foam under the projection of the light source.The learning curve is short,which is convenient for flexible use at all levels of hospitals. |
