BackgroundIn 2014,the National Health Comission and other 6 departments issued the "Opinions on the Collaboration of Medical Training and Education to Deepen the Reform of the Development of Talents with Clinical Medical Degree.The opinion clearly proposed full-time graduates studying for the clinical medical professional degree participate in standardized training for residents.Full-time graduates studying for the clinical medical professional degree perceive great stress due to factors such as lack of clinical skills,inadequate clinical experience,lack of communication skills,employment stress,economic stress and marriage stress.Social support can change their views on stressful events and help them master the corresponding methods of pressure to reduce their perceived stress.Therefore,it is of great significance to study the impact of social support on the perceived pressure of full-time graduates studying for the clinical medical professional degree.ObjectivesThe objectives are to inquire the status quo of perceived stress and social support of full-time graduates studying for the clinical medical professional degree,analyze the factors that may affect perceived stress and social support and to explore the influence of social support on perceived stress.MethodsThe study applied the convenience sampling method.From January to April 2019,a questionnaire survey was conducted in 9 standardized training bases for residents in Shandong Province.The questionnaire consists of three parts:the basic information of respondents,perceived stress scale and social support assessment scale.At the same time,the perceived stress scale scores and the social support assessment scale scores were analyzed as continuous variables and converted categorical variables.As an analysis process of continuous variable,the total scores of the perceived pressure scale,the total scores of the social support assessment scale,and the scores of each dimension of the two scales are described in the form of mean and standard deviation.For full-time graduates studying for the clinical medical professional degree with different sociodemographic characteristics,the differences in the results of perceived stress scale and social support assessment scale were compared using independent sample t-test and one-way analysis of variance.Because the perceived stress scale scores and the social support assessment scale scores do not obey the normal distribution,spearman correlation analysis was used to study the correlation between the perceived stress scale scores of full-time graduates studying for the clinical medical professional degree and the social support assessment scale scores.In the analysis process of converted categorical variable,our classification criteria are as follows:the pressure perception scale score is 26 and above,which is judged as a health risk stress;the 27%of the observation value of the total score of the social support assessment scale is the cutoff value to determine high,middle,and low social support.Chi-square test was used to analyze whether the proportion difference of health risk stress state of full-time graduates studying for the clinical medical professional degree under different socio-demographic characteristics is significant or not,and whether the proportion difference of high,middle,and low social support of full-time graduates studying for the clinical medical professional degree under different socio-demographic characteristics is significant or not.The chi-square correlation test was used to analyze the correlation between high,medium,and low social support and health risk stress state.Multivariate logistic regression analysis was used to determine deeply whether subjective support,objective support,and the utilization of support as the three dimensions of social support assessment scale will affect the perceived pressure of full-time graduates studying for the clinical medical professional degree under the control of covariates.Results and DiscussionA total of 1062 full-time graduates studying for the clinical medical professional degree were included in this study.The average age of the respondents was 27.3 years.The average score of the perceived pressure scale of full-time graduates studying for the clinical medical professional degree is 25.86,which is larger than the norm of 24.22.The full-time graduates studying for the clinical medical professional degree under health risk stress state account for 55.7%of the total sample,which is also larger than the norm of 44.54%.Full-time graduates studying for the clinical medical professional degree feel higher stress than the general population.The average score of the social support assessment scale for full-time graduates studying for the clinical medical professional degree is 35.91,which is larger than the norm of 34.56.The full-time graduates studying for the clinical medical professional degree feel higher social support than the general population.Hometown(rural or urban areas),the average monthly income and the number of management beds will affect the score of the perceived pressure scale of full-time graduates studying for the clinical medical professional degree significantly.Hometown(rural or urban areas),the average monthly income,the number of management beds,and the discipline of professional training all affect whether full-time graduates studying for the clinical medical professional degree are in a state of health risk stress or not.Gender,hometown(rural or urban areas),the discipline of professional training,monthly average income and the number of management beds all affect the scores of social support assessment scale for full-time graduates studying for the clinical medical professional degree.Hometown(rural or urban areas),the monthly average income and the number of management beds will affect whether full-time graduates studying for the clinical medical professional degree are in high social support,middle social support or low social support.The analysis results of perceived pressure scale and the social support assessment scale as continuous variables are not completely consistent with the analysis results of converted categorical variables.Since the result indicators after conversion to categorical variables are more differentiated,we agree with the results after conversion to categorical variables.Our tendency also needs to be further verified by future research.The results of Spearman correlation analysis showed that the perceived stress scale score was moderately correlated with the social support assessment scale score,and the correlation coefficient is-0.432.The results of the chi-square correlation test show that high,middle,and low social support was moderately correlated the state of health risk stress,and the correlation coefficient is 0.357.The multivariate logistic regression analysis demonstrates that under the control of concomitant variables such as the hometown(rural or urban areas),the discipline of professional training,the average monthly income and the number of bed management,increasing subjective support,objective support,and utilization of support can all reduce the risk of health risk stress.Conclusion and SuggestionsThe perceived pressure and social support of full-time graduates studying for the clinical medical professional degree both are higher than those of the general population.Perceived pressure is moderately related to social support.The greater the social support,the lower the perceived pressure.Increasing subjective support,objective support,and utilization of support can all reduce the risk of health risk stress.In addition,we should also increase financial subsidies for full-time graduates studying for the clinical medical professional degree,and reasonably set the workload of full-time graduates studying for the clinical medical professional degree according to specific training discipline to reduce perceived pressure.
