Objective: The purpose of this study was to treat malignant blood stasis type primary dysmenorrhea with moxibustion of Shen Que,Guan Yuan point combined with auricular point sticking,and to set an oral western medicine group as a control to evaluate the clinical efficacy of the two treatment methods in treating blood stasis type primary dysmenorrhea.We will discuss the causes and analyze them in detail,in order to provide a simple and effective new idea for clinical treatment of primary dysmenorrhea.Methods: Sixty-seven patients with primary dysmenorrhea who met the inclusion criteria were randomly divided into moxibustion combined with auricular acupressure group(34 cases)and western medicine treatment group(33 cases).In the final analysis,32 patients were included in the moxibustion combined auricular acupressure group and 30 in the western medicine treatment group.Moxibustion combined with auricular point sticking and pressing group adopted the following treatment methods: moxibustion are taken from Shen que point and Guan yuan point,starting treatment 7 days before each menstrual cycle,moxibustion once a day,30 min each time,7 consecutive days for a course of treatment;Auricular acupoint are CO18,TF2,CO10,CO12,AT4 and TF4,treatment is started 7 days before each menstrual cycle,Auricular acupoints are changed once every other day,and left and right ears are alternated,Pressure treatment continued until the end of the second day after menstrual cramps;a total of three menstrual cycles were treated.Visual analogue scale of pain(VAS)and COX symptom scale of dysmenorrhea(CMSS)were used as the clinical observation indicators,the relevant evaluations were performed before treatment,after the end of the 3rd menstrual cycle treatment,and at the 3rd month after the end of treatment,and the final clinical efficacy was compared between the two groups.Results:(1)The clinical efficacy of the two groups was compared.There were 32 patients in the moxibustion combined with auricular point sticking group,of which 4 cases were cured,18 cases were markedly effective,9 cases were effective,1 case was ineffective,and the total effective rate was 96.88%.In the western medicine treatment group,a total of 30 patients were accepted relevant treatment,0 were cured,8 were markedly effective,18 were effective,and 4 were ineffective.The total effective rate was 86.67%.By the analysis of rank sum test,the efficacy of the two groups was compared with P < 0.01,showing a significant difference.(2)Visual analogue scale(VAS)of pain was compared between the two groups,and the difference between the two groups after 3 cycles of treatment and before treatment was statistically significant(P<0.05).Compared with the western medicine treatment group,the difference of moxibustion combined with auricular point pressing group was statistically significant(P<0.05).During the follow-up period,there was no statistically significant difference in VAS scores between the moxibustion combined with auricular point pressing group and the post-treatment group(P > 0.05).VAS scores in the western medicine treatment group were statistically significant(P<0.05)compared with those after 3 cycles of treatment,and VAS scores in the follow-up period were close to the level before treatment.(3)The two groups were compared in terms of the duration of dysmenorrhea scale(COX1)score,and the differences between the two groups were statistically significant after 3 cycles of treatment compared with before treatment(P<0.05).Compared with the western medicine treatment group,the difference of moxibustion combined with auricular point pressing group was statistically significant(P<0.05).During the follow-up period,there was a statistical difference in the COX1 score between the moxibustion combined with ear point pressing group and the group after treatment(P < 0.05),and the score continued to decline.There was a statistically significant difference in the COX1 score between the western medicine treatment group and the treatment group after 3 cycles(P<0.05),and the COX1 score during the follow-up period was close to the level before treatment.(4)The two groups were compared in terms of the COX2 score of dysmenorrhea symptom severity scale.The differences between the two groups were statistically significant after 3 cycles of treatment compared with before treatment(P<0.05).Compared with the western medicine treatment group,the difference of moxibustion combined with auricular point pressing group was statistically significant(P<0.05).During the follow-up period,there was a statistically significant difference in the COX2 score between the moxibustion combined with ear point pressing group and the group after treatment(P < 0.05),and the score continued to decline.There was a statistically significant difference in COX2 score between the western medicine treatment group and the treatment group after 3 cycles(P<0.05),and the COX2 score at the follow-up period was close to the level before treatment.Conclusion:(1)The two treatment methods of moxibustion Shen Que,Guan Yuan combined with auricular point pressing and oral ibuprofen sustained-release capsules can effectively treat primary dysmenorrhea of blood stasis type.(2)In terms of short-term efficacy,moxibustion combined with auricular point sticking is superior to oral western medicine in improving the degree of abdominal pain,the duration of dysmenorrhea and the severity of dysmenorrhea.(3)In terms of long-term therapy,the treatment method of moxibustion combined with auricular point sticking has a good long-term effect,which can not only maintain the analgesic effect at the level after the treatment,but also continuously improve the duration and severity of dysmenorrhea symptoms after the treatment.Oral western medicine treatment does not have the corresponding long-term effect.(4)The overall clinical efficacy of moxibustion combined with auricular point pressing is better than that of oral western medicine.
