| Objective:Through the clinical study of Huayu Xiaoban Decoction in the treatment of children’s purpura nephritis(yin deficiency and Huowang type),observe the clinical effect of Huayu Xiaoban Decoction in the treatment of this disease,provide the clinical basis for further clinical application of Huayu Xiaoban Decoction in the treatment of SHPN(hair Yin deficiency and fire flourishing type),and observe and explore the clinical effect and significance of the combination of Chinese and Western medicine in the diagnosis and treatment of this disease.Methods: From September 2017 to December 2019,60 children with Henoch Schonlein purpura nephritis(yin deficiency and fire hyperactivity type)who met the inclusion criteria were collected from the General Hospital of Changchun University of traditional Chinese Medicine Affiliated Hospital and two pediatric clinics.The children were randomly divided into experimental group and control group with 30 cases.The control group was given rutin tablets,Persantine and vitamin C tablets The experimental group was treated with Huayu Xiaoban Decoction and Western medicine.The treatment time of this clinical study is 8 weeks.Follow up for 3 months and record the recurrence of the two groups during the follow-up period.Results:(1)General situation comparison: before treatment,there was no significant difference in age,gender,course of disease,degree of illness,TCM syndrome(main syndrome),TCM syndrome(secondary syndrome),experiment related biochemical indicators and safety indicators between the two groups(P > 0.05),which was comparable(2)The total curative effect of the two groups: 30 cases in the treatment group,27 cases of total effective,3 cases of ineffective,the total effective rate of 90.00%.In the control group,there were 30 patients,21 patients were effective,9 patients were ineffective,and the total effective rate was 70.00%.There was a significant difference between the two groups(P < 0.05).The effect of the treatment group was better than that of the control group.(3)Comparison of the total score of TCM syndromes in the two groups:comparison within the group: after treatment and before treatment,the total score of TCM syndromes in the two groups were significantly improved,the difference was statistically significant(P < 0.05).Comparison between groups: after treatment,there was a significant difference in the total score of TCM syndrome(P < 0.05),and the improvement of the treatment group was better than that of the control group.(4)Comparison of curative effect indexes between the two groups: comparison within the group: after treatment and before treatment,the urinary protein,urinary sediment red blood cell count,RBC(Gao Bei)number and 24 h urinary protein quantity of the two groups were significantly improved,the difference was statistically significant(P<0.05).The difference of urine protein,RBC count and 24-hour urine protein between the two groups was statistically significant(P < 0.05).The improvement of the treatment group was better than that of the control group.(5)Comparison of recurrence rate between the two groups: after the treatment,during the follow-up period,in the treatment group,3 cases recurred,the recurrence rate was 10.00%;in the control group,10 cases recurred,the recurrence rate was 33.33%;there was significant difference between the two groups(P < 0.05),the recurrence rate in the treatment group was significantly lower than that in the control group.(6)Safety index comparison: there was no significant change in liver function(alt,AST)and renal function(SCR,BUN)before and after treatment and during follow-up.In addition,some children in the two groups had slight adverse reactions during the treatment,but the children could tolerate them without other special symptoms and obvious adverse effects.Conclusion:(1)Huayu Xiaoban decoction can effectively control and improve the clinical symptoms of children.(2)Huayu Xiaoban decoction can effectively reduce the main therapeutic indexes of the disease,and significantly reduce the recurrence rate of the disease.(3)The diagnosis and treatment of this disease with the combination oftraditional Chinese medicine and Western medicine has the advantages that traditional Chinese medicine or western medicine alone does not have. |
