Objective: To understand the occurrence,transfusion treatment and prognosis of anemia in preterm infants with gestational age ? 34 weeks,and to clarify the relationship between each influencing factor and anemia in preterm infants,so as to provide basis for clinical prevention and treatment of anemia in preterm infants.Methods: The data of 698 preterm infants with gestational age ? 34 weeks in our neonatal intensive care unit from January 1,2017 to December 31,2018 were analyzed retrospectively,including 258 anemic preterm infants and 440 non anemic preterm infants.698 preterm infants were divided into non anemia group(n=440)and anemia group(n=258)according to whether anemia occurred or not.Relevant data were collected,and single factor analysis was conducted for the relevant high-risk factors.The significant factors in single factor analysis were included in the logistic binary regression for multi factor analysis,and the independent risk factors of anemia in preterm infants with gestational age ? 34 weeks were identified.Results: 258 of 698 preterm infants with gestational age ? 34 weeks had anemia,the incidence of anemia was 36.96%,149 of them received blood transfusion,the incidence of blood transfusion was 57.75%.The smaller the gestational age was,the lighter the birth weight was,the higher the incidence of severe preterm infants was.In the single factor analysis,the gestational age,birth weight,NRDS,asphyxia,use of pulmonary surfactant,mechanical ventilation time,sepsis There was significant difference between the two groups in nine high risk factors of iatrogenic blood loss and basic hemoglobin during hospitalization(P<0.05),but there was no significant difference between the two groups in pregnancy with severe eclampsia,fetal distress and basic hematocrit(P>0.05).In logistic regression analysis,six factors,including asphyxia,gestational age,birth weight,basic hemoglobin level,mechanical ventilation time and iatrogenic blood loss during hospitalization,were independent risk factors for anemia in preterm infants with gestational age ? 34 weeks.Conclusion: Premature anemia is affected by many factors.The smaller the gestational age,the lower the birth weight,the lower the basic hemoglobin,the longer the time of birth with asphyxia,the longer the time of mechanical ventilation,the more the iatrogenic blood loss,the greater the risk of premature anemia.Early identification of premature anemia,early improvement of anemia,reduction of venous blood collection are helpful to reduce the risk of premature anemia and improve it Prognosis of premature infants.
