Meta-analysis Of Yiqi Yangyin Bushen Huoxue Therapy For The Treatment Of Diabetic Nephropathy Stage Ⅲ

Objective: Meta-analysis was used to evaluate the effectiveness and safety of oral Yiqi Yangyin Bushen Huoxue traditional Chinese medicine decoction for treating stage Ⅲ of diabetic nephropathy.Methods: The computer was retrieved from China How Net Database(CNKI),Wanfang Database,Chongqing Weipu Chinese Science and Technology Periodicals Database(VIP),China Biomedical Literature Database(CBM),Pub Med database,and Cochrane library.The retrieval dates were from January 2000 to December 2019.The patients with stage Ⅲ diabetic nephropathy were used as the research object,and randomized controlled trials on the treatment of stage Ⅲ of diabetic nephropathy with Yiqi Yangyin Bushen Huoxue traditional Chinese medicine decoction were collected.Formulate the inclusion and exclusion criteria.Two researchers should conduct literature screening according to the corresponding acceptance criteria,search the literature entries and enter Endnote X9,exclude duplicate documents,and then read the titles and abstracts to exclude documents that do not meet the requirements of this research(including animal experiments,Cases,non-randomized controlled trials,etc.),and then read the full text to screen the literature that may meet the inclusion criteria,and obtain the literature that meets the requirements.For the included system literature,the Cochrance Collaboration Network bias assessment risk tool was used for risk bias assessment.At the same time,the corresponding data and information were extracted from the table.Revman5.3 software was used for qualitative description analysis or quantitative meta analysis.Finally,the results of the system review Explain and discuss.Results: After relevant searches,a total of 4,766 articles were retrieved.Finally,15 articles were included in the systematic review.A total of 1468 patients were included,including 768 patients in the test group and 700 patients in the control group.Meta analysis results show that: the experimental group of Chinese medicine Yiqi Yangyin Bushen Huoxue method combined with western medicine basic treatment in the treatment of diabetic nephropathy stage III in terms of total clinical effectiveness [OR = 3.69,95%CI(2.68,5.09),P <0.00001],Decrease the total protein in urine[SMD =-2.19,95% CI(-3.25,-1.13),P <0.0001)],blood urea nitrogen [MD =-0.99,95% CI(-1.74,-0.24),P < 0.00001],urinary protein excretion rate [SMD =-1.23,95% CI(-1.69,-0.77),P<0.00001] and other aspects are better than the control group using western medicine alone than the western medicine treatment alone,reducing blood creatinine [ MD =-8.81,95% CI(-18.89,1.27),P =0.09],there is no significant difference between the two groups;in reducing fasting blood glucose [MD =-0.87,95% CI(-1.18,-0.56),P < 0.00001],TCM Yiqi Yangyin Bushen Huoxue method combined with western medicine basic treatment is better than western medicine treatment alone.It lowers blood glucose 2 hours after a meal [MD=-0.77,95% CI(-1.72,-0.19),P = 0.12 ],Glycated hemoglobin [MD=-0.54,95% CI(-1.07,-0.00),P = 0.05],there is no significant statistical difference between the two groups;in reducing total cholesterol [MD =-0.95,95% CI(-1.48,-0.41),P = 0.0005],triglycerides [MD =-0.51,95% CI(-0.68,-0.33),P <0.00001] Better than the use of Western medicine.Research conclusions: Using traditional Chinese medicine to nourish qi,nourish yin and nourish kidney and promote blood circulation in the treatment of diabetic nephropathy in stage III is more effective in improving the total clinical effectiveness and reducing the 24-hour urine protein quantification,blood urea nitrogen,urine protein excretion rate,fasting blood glucose,triglycerides,total cholesterol Western medicine is more effective,and there are no significant differences in reducing blood creatinine,2 hours postprandial blood glucose,and glycated hemoglobin.However,considering that the quality of the literature included in this study is low,there are methodological defects,and the strength of evidence is poor.It needs to be further supported by further large-sample,multicenter,long-term follow-up randomized controlled trials.
