| Objective:1.To compile the Questionnaire on the KAP of Clinical Nursing Staff on Humanistic Care,to provide assessment tools for medical institutions to carry out the investigation on the status quo of the KAP of humanistic care of clinical nursing staff.2.To evaluate the status quo of humanistic care of clinical nursing staff,ai.J analyze its influencing factors to provide theoretical basis for improving the humanistic care ability of clinical nursing.Methods:First of all,Delphi method was used to conduct two rounds of expert letter consultation on the self-compiled questionnaire on humanistic care of clinical nursing staff and develop the initial questionnaire.The initial questionnaire was used to conduct pre-survey on 388 clinical nursing sta?:,and 40 respondents were selected for retest 2 weeks later.The questionnaire was adjusted according to the results and the formal questionnaire was established.Secondly,2025 clinical nurses from 5 grade A hospitals in Shandong province were selected to investigate the status quo of humanistic care.SPSS 22.0 software was used for statistical analysis,and standard 'difference,mean and percentage were used for descriptive analysis.Independent-Sample t Test or ANOVA were used to compare the measurement data.Pearson correlation analysis and multiple stepwise regression were used to explore the influencing factors of KAP of clinical nurses on humanistic care.Results:1.Results of Delphi expert inquiries and preliminary investigationThrough two rounds of expert inquiries,the preliminary "Questionnaire on the KAP of Clinical Nursing Staff on Humanistic Care " was formed.The items included three dimensions:knowledge,attitude and practice.Among them,the knowledge dimension contained 18 items,the attitude dimension contained 22 items,and the practice dimension contained 23 items,totally 63 second-level items.The positive coefficients of experts in the two rounds were 100.0%and 96.4%,the authoritative coefficients of experts were 0.888 and 0.870,and Kendall ' s W were 0.342 and 0.467 respectively(P<0.05).I-CVI was 0.889?1.000,and S-CVI was 0.980.It shows that the result of letter inquiry is scientific,reasonable and reliable.Pre-survey data show that the total Cronbach's a of the questionnaire is 0.982,the Cronbach's a of each dimension is 0.955?0.983,the overall retest reliability coefficient of the questionnaire is 0.983,and the retest reliability coefficients of each dimension are 0.952?0.988.It shows that the internal consistency and stability of the questionnaire are good,and it can be used as a formal questionnaire.2.Results of investigation on the status quo of humanistic care knowledge,practice end practice of clinical nursing staffThe results of general demographic data of clinical nurses were as follows:female-dominated,1923(95.0%);1006(49.7%)under 30 years old,805(39.8%)between 30 and 40 years old,214(10.6%)over 40 years old;1427(70.5%)nurses with working experience of 10 years or less,411(20.3%)nurses with 11?20 years,and 187(9.2%)nurses with working experience of more than 20 years;bachelor's degree was the main educational background at present,1711(84.5%)nurses;primary titles accounted for 1,447 people(71.5%);the majority of nurses were employed by contract system 1,452(71.7%).The total score of humanistic care KAP of clinical nursing staff and the score of three dimensions:the average score of KAP on humanistic care(259.54±33.55),the standard score(82.39±10.65),the average score of knowledge dimension(68.16±13.43),the standard score(75.74±14.92),the average score of attitude dimension(90.32±13.81),and the standard score.Accurate score(82.11±12.55),average score of practice dimension(101.05±12.94)and standard score(87.87±11.25).Univariate analysis results showed that clinical nursing staff with different gender,age,reproductive status,relationship with parents,employment reasons,initial education background,department,working years,title,position,employment mode,night shift frequency had statistically significant differences in the total score of humanistic care,knowledge,practice and practice(P<0.05).Clinical nursing staff with different age,fertility status,relationship with parents,employment reasons,initial educational background,working years,professional title,position,employment mode and night shift frequency had statistically significant differences in scores of humanistic care knowledge dimensions(P<0.05).Clinical nursing staff with different age,fertility status,relationship with parents,employment reasons,initial educational background,working years,professional title,position,employment mode and night shift frequency had statistically significant differences in scores on humanistic care belief/attitude dimensions(P<0.05).Clinical nursing staff with different gender,age,family of origin,marital status,fertility status,relationship with parents,employment reasons,initial education background,department,working years,title,position,employment mode and night shift frequency had statistically significant differences in scores of humanistic care behavior dimensions(P<0.05).Pearson correlation analysis showed that knowledge,belief/attitude and practice scores were positively correlated,and the correlation coefficients r were 0.638,0.431 and 0.560,respectively,and P<0.001.Multiple stepwise regression analysis shows that:The influencing factors of KAP total score of clinical nursing staff on humanistic care were fertility status(?=0.073,P<0.05),and the relationship between the parents(?=0.137,P<0.05),employment reason(?=0.060,P<0.05),the initial degree(?=-0.101,P<0.05),the department(P=0.059,P<0.05),post(?=0.080,P<0.05),employment way(?=0.071,P<0.05),night shift frequency(?=-0.093,P<0.05).The score of knowledge dimension was affected by fertility status(?=0.105,P<0.05),relationship with parents(?=0.104,P<0.05),position {?=0.075,P<0.05),employment mode(?=0.065,P<0.05),night shift frequency(??-0.094,P<0.05).The influencing factors of belief/attitude dimension score were the relationship with parents {?=0.114,P<0.05),position(?=0.116,P<0.05),employment reason(?=0.060,P<0.05),night shift frequency(?=-0.064,P<0.05).Behavioral dimension scores were correlated with gender(?=0.071 P<0.05),relationship with parents(?=0.134,P<0.05),initial education level(?=-0.140,P<0.05),department(?=0.074,P<0.05),mode of employment(?=0.072,P<0.05),night shift frequency(?=-0.064,P<0.05).Conclusions:1.The self-compiled questionnaire has a good reliability and validity,which can be used as a survey too]to evaluate the status quo of humanistic care of clinical nurses.2.The current situation of knowledge,attitude and practice of humanistic care among clinical nurses is in the middle level(the standard score is 60-85).It shows that the overall level of clinical nurses' knowledge,attitude and practice of humanistic care still has room for further improvement.3.Fertility status,relationship with parents,employment reasons,initial educational background,department,position,employment mode and night shift frequency are the influencing factors of the KAP current situation of clinical nursing staff on humanistic care.Among them,the initial educational background and night shift frequency of nursing staff are negatively correlated with the status quo of humanistic care.Therefore,medical institutions should adopt targeted and focused training methods and intervention measures to improve the level of nursing staffs KAP from different influencing factors of clinical nursing staffs humanistic care. |
