Study On Complications And Its Influencing Factors Among 2,136 Pregnant Women In Lhasa

ObjectiveIn recent years,with the opening of the comprehensive two-child policy in China,the incidence of complications with various forms in pregnancy has been increasing,which has brought many adverse effects on maternal and fetal health statuses,and even their lives could be endangered.This study sought to analyze the prevalence and its influencing factors of pregnancy complications among Tibetan pregnant women,and to provide reference basis for the prevention and control of pregnancy complications.MethodsA total of 2136 pregnant women who hospitalized and delivered in Lhasa people’s Hospital were selected in this study from September 1,2017 to August 31,2018.A retrospective investigation of the medical records was carried out.The information of general demographics,history of pregnancy and childbirth,pregnancy status,and morbidity of pregnancy complications were collected from pregnant women in the survey.Epidata 3.0Software was used for data inputting.SPSS 20.0 Software was adopted to analyze the inputted data,and to make descriptive analyses of characteristics for the pregnant women.In addition,χ~2 test and binary stepwise logistic regression model were applied to explore the related factors associated with pregnancy complications.Results1.The pregnancy condition of Tibetan women in LhasaAmong included pregnant women,84.6%were within appropriate age.The proportion of pregnant women who took folic acid,iron,calcium were 39.9%,21.3%and 27.6%,respectively.Prenatal cares among 57.3%pregnant women were less than 7 times.2.The incidence of pregnancy complicationsAmong 2136 respondents,a total of 1415 cases of pregnancy complications were detected,with a detection rate of 66.2%.The top five pregnancy complications were Anemia(33.8%),premature rupture of membranes(14.7%),hypertension(10.8%),vaginitis(9.3%),and hepatitis B(9.0%),respectively.3.Influencing factors of pregnancy complicationsResults of multiple stepwise logistic regression analysis showed that pregnancy complications in Tibetan pregnant women were associated with elderly pregnant women,previous two history of miscarriage and above,previous history of cesarean section,remembering the last menstruation,and multipara.There was no statistically significant differences in the prevalence of pregnancy complications between low and appropriate age,and pregnant women who aged 35 and above(OR=2.276,95%CI:1.048~4.942)had a higher risk of pregnancy complications.Compared with pregnant women who forget their last period,the pregnant women who remembered their last period(OR=1.463,95%CI:1.132~1.891)had a higher risk of pregnancy complications.Compared to the pregnant women who had no previous history of cesarean section,Pregnant women who had previous history of cesarean section(OR=1.699,95%CI:1.168~2.473)had a higher risk of pregnancy complications than those who without history of cesarean section.Compared to pregnant women who had one previous miscarriage,those who had two times of miscarriages and above(OR=2.083,95%CI:1.084~4.003)were more likely to have pregnancy complications.Compared to the primipara,pregnant women with no previous delivery(OR=0.612,95%CI:0.484~0.775)and two previous delivery and above(OR=0.571,95%CI:0.400~0.815)had a lower risk of pregnancy complications.Conclusions and SuggestionsThe antenatal examination times,folic acid intake rate,iron intake rate and calcium intake rate among Tibetan pregnant women in Lhasa during pregnancy were all at relatively low levels.The incidence of pregnancy complications was high,and Anemia,premature rupture of membranes,hypertension,vaginitis and hepatitis B were the main pregnancy complications.Elderly pregnant women,previous two history of miscarriage and above,previous history of cesarean section,remembering the last menstruation were risk factors for pregnancy complications,and multipara was protective factors for pregnancy complications.Therefore,the work for the pregnant health care and health guidance of Tibetan pregnant women in Lhasa should be strengthened,Increased vigilance against risk factors of pregnancy complications should be applied to prevent and control pregnancy complications at the source,and to reduce the prevalence of pregnancy complications of Tibetan pregnant women.
