| Objective: To explore the effectiveness of peer support in improving self-management compliance in patients with Diabetes Ketoacidosis(DKA)in qianbei region,and to provide a scientific basis for clinical intervention to help DKA patients to shape healthy behaviors.Methods: According to the exclusion criteria,72 patients with mild to severe DKA who were admitted to the endocrinology department of three tertiary public hospitals in qianbei region from May 2017 to April 2018 were selected as subjects.Patients were divided into control group(36 cases)and test group(36 cases)by random number table method.The control group was routinely discharged from the hospital,and the experimental group was given peer support intervention on the basis of the control group.At the 3rd and 6th month after the intervention,the Diabetes Management Self-Efficacy Scale(DMSES)and the Summary of Diabetes Self-management Activities(SDSCA)were used to investigate the patient’s self-efficacy and self-management behavior and monitored the patient’s Fasting plasma-glucose(FPG),2-hour post-meal blood glucose(2hPG)levels and Glycosylated hemoglobin A1(HbA1c)compliance rate.Results:1.General information of patients : There were no differences in gender,age,marital status,Body Mass Index(BMI),type of Diabetes,duration of disease,degree of illness,predisposing factor,education level,occupation,type of medical insurance and family income per capita(P>0.05).2.Before the intervention,the total scores of self-efficacy,self-management behavior and scores of each group,FPG,2hPG and HbA1 c were compared between the two groups(P>0.05).3.DMSES scores : After 3 months of intervention,the total scores of self-efficacy and the scores of all dimensions(except healthy diet)were compared between the two groups(P<0.05).After 6 months of intervention,the total scores of self-efficacy and the scores of all dimensions(except healthy diet,diet and blood glucose)were compared between the two groups(P<0.05).4.SDSCA scores : After 3 months of intervention,the total scores of self-managementbehavior and scores of all dimensions(except diet management and foot care)were compared between the two groups(P<0.05).After 6 months of intervention,the scores of self-management behavior and scores of all dimensions(except diet management and exercise management)were compared between the two groups(P<0.05).5.Diabetes blood glucose monitoring indicators : After 3 months of intervention,the FPG level and HbA1 c compliance rate of the experimental group were better than the control group(P<0.05),and the 2hPG levels in the two groups were compared(P>0.05).After 6months of intervention,the FPG level and HbA1 c compliance rate of the experimental group were better than the control group(P<0.05),and the 2hPG levels of the two groups were compared(P>0.05).Conclusion: Applying peer support education to the self-management behavior of regionalized DKA patients has achieved certain effects on improving patient self-management efficacy and self-management behavior compliance,it has laid a certain foundation for the construction of an education management model that conforms to the characteristics of the hospital-community-patient “trinity”. |
