| 【Objective】Based on the biological nurturing theory,this paper discusses the effect of laidback breastfeeding posture on lactation related nipple pain during postpartum hospitalization,clarifies the relationship between nipple pain and early postpartum feeding,and explains the effect of laid-back breastfeeding posture on breastfeeding rate and breastfeeding self-efficacy within 42 days after delivery.In order to improve the rate of exclusive breastfeeding,provides a reliable technical scheme.【Methods】From July 2018 to August 2018,152 cases of eligible participants women in Haikou Maternal And Child Health Care(Haikou,Hainan)were selected as the research subjects.While in hospital,they were divided randomly into two groups,the experimental group received laid-back position guidance,and the control group received standard position guidance.Follow-up after discharge was performed using WeChat."the First latch",nipple pain,breastfeeding status,breastfeed self-efficacy were evaluated for statistical analysis,and SPSS 23.0 was used for data analysis.【Results】1.A total of 144 cases were collected in this study,including 70 cases in the control group and 74 cases in the experimental group.Baseline equilibrium was comparable in age,gestational age,delivery mode,education level,neonatal weight,neonatal gender,expected feeding mode,and prenatal breastfeeding self-efficacy between the two groups(P>0.05).2.Results of clinical intervention2.1 The success rate of "the First latch" in the experimental group was(86.5%)higher than that the control group(70.0%),and the difference between the two groups was statistically significant(P<0.05).2.2 A total of 2948 lactation related nipple pain scores were recorded in the two groups within 3 days of delivery,including 1529 in the experimental group and 1419 in the control group.The incidence of nipple pain on the day of delivery(21.0% vs.32.1%),the second day(27.9% vs.54.5%)and the third day(24.0% vs.53.0%)was significantly different between the two groups(P<0.05).2.3 There were statistically significant differences in the degree of nipple pain on the day of delivery,the second day and the third day(P<0.05),includes self-rating and midwife evaluation scores.The average rank of pain scores of the experimental group was lower than that of the experimental group all the three days,and the intervention had a protective effect on the degree of nipple pain.2.4 There were strong positive correlation between the degree of nipple pain and feeding mode at 7 days postpartum,and a weak positive correlation at 42 days postpartum.2.5 The breastfeeding status of the experimental group was better,and the difference between two groups was statistically significant(P<0.05).The exclusive breastfeeding rate at 7 days postpartum in the experimental group(63.5%)was higher than that in the control group(40%).The rate of exclusive breastfeeding at 6 weeks postpartum in the experimental group(59.5%)was higher than that in the control group(42.9%),the difference between the two groups was statistically significant(P<0.05).2.6 There were no statistically significant difference in self-efficacy scores of breastfeeding between the two groups(P>0.05),but the breastfeeding self-efficacy scores change with the time difference was statistically significant(P<0.05),the two groups in time and group factors interaction effect difference have statistical significance(P<0.05).【Conclusions】Laid-back breastfeeding position is conducive to the correct latch of newborn,promote early lactation model to establish normal,effectively reduce the incidence of nipple pain as well as the degree of primipara sore nipples,produce positive effects on postpartum breastfeeding self-efficacy to improve,improving primipara breastfeeding rates during hospitalization and postpartum 42 days. |
