Objective: Preeclampsia is an idiopathic disease during pregnancy,which is one of the important causes of maternal death and adverse fetal outcomes.The mortality rate of preeclampsia is among the top three among obstetrical complications.The etiology and pathogenesis of preeclampsia have not yet been fully elucidated.If early detection of high-risk pregnant women with preeclampsia and active and effective treatment,it is of great significance to prevent preeclampsia from improving pregnancy outcome.In recent years,researchers at home and abroad have been working to find sensitive indicators related to the pre-eclampsia episode.Endocan is one of the concerns,but the results of each study have not been agreed.The purpose of this study was to systematically evaluate the association of blood endocan levels with preeclampsia using an evidence-based approach.Methods: We search the Chinese database of CNKI,Wanfang,CBM and VIP electronic databases and the English database of PubMed,The Cochrane Library,and EMBASE(OVID)by computer to search for domestic and international published research reports on the relationship between blood endocan levels and preeclampsia.References included in the literature,the literature search time is built until May 1,2018.Two evaluators independently screened the literature,extracted the data in strict accordance with the established inclusion and exclusion criteria,and used the AHRQ scale to assess the risk of bias in the included studies.The meta-analysis was performed using RevMan5.3 software.The combined effect size index was mean±standard deviation(mean±SD).The selection of the combined effect model depends on the corresponding heterogeneity test results,and further adopts the method of elimination one by one analysis,finally,funnel plot was drawn to evaluate publication bias.Results:1.A total of 8 articles were included,one of which involved two studies on the relationship between blood endocan levels and preeclampsia.Therefore,a total of 9 cross-sectional studies were conducted with meta-analysis.The quality of the literature was based on the AHRQ scale: “Yes” has the highest proportion and “unclear” has the lowest proportion,indicating that the quality of the included documents is high.2.Nine studies related to the relationship between blood endocan levels and preeclampsia risk,including 436 cases in the case group and 388 cases in the control group.The results showed that there was a statistically significant correlation between blood endocan levels and preeclampsia.(MD=0.82,95% CI:0.31-1.34,P=0.002)and(MD=1.10,95%CI:0.51-1.68,P=0.0002)3.Three studies involving the association of blood endocan levels with preeclampsia severity showed no statistically significant association between endocan levels and preeclampsia severity.(MD=0.36,95% CI=-5.76-6.47,P>0.05)Conclusions:There is a correlation between the level of blood endocan and the pre-eclampsia,but there is no correlation with the severity of pre-eclampsia.However,due to the limited quality of existing inclusion studies,subgroup analysis of different ranges of endocan levels is not possible,so high-quality prospective cohort studies are needed to provide more reliable evidence for further validation.
