The Construction Of Index System Of Core Competence For Nursing Professionals With Combination Of Medical And Health Care

Objective Based on the definition of the nursing professionals with combination of medical and health care,this study initially constructed the index system of core competence for nursing professionals with combination of medical and health care,and then determined the weights of indicators at all levels,finally conducted a test of the reliability and validity of the index system to explore the practical application of the index system.The study aimed to provid a theoretical basis for effectively responding to the reality of the rapid advent of aging society and the lack of nursing professionals,and how to train,appoint,assess and evaluate the nursing professionals.Methods By useing the methods of literature review,theoretical analysis and expert interview,the basic theories and principles of the index system of core competence for nursing professionals with combination of medical and health care were discussed,and the items of indicators were preliminarily constructed.Through the Delphi erpert consulation method,the initial indicator system was modified and screened to determine the entry of the index system.The Analytic Hierarchy Process(AHP)method was used to calculate the weights of level of indicators by yaahp.And based on the constructed indicator system,327 nurses in Pilot units of Shanxi province were investigated to text the reliability and validity of the index system.The Cronbach's ? coefficient and split-half confidence were used to test the reliability of the index system,content validity index was used to text the content validity,the exploratory factor analysis was used to text the structural validity,and the Person conrrelation coefficient was used to text the correlation among the dimensions of the index system and between each dimension and the index system.Results1.Preliminarily constructed the index system of core competence for nursing professionals with combination of medical and health care24 erperts completed two rounds of expert consulation in this study.The recovery rates of the questionnaire were 86% and 80%,respectively,and the effective recovery rates were both 100%,indicating that the experts were motivated.The results of two rounds of consulation showed :(Ca)=0.925,(Cs)=0.738,(Cr)=0832(>0.70),indicating that the authority of the experts was high.The coefficient of variation(CV)of two rounds were between 0.05~0.24 and 0.00~0.22,and the Ken-dall's W were 0.230 and 0.268,respectively,indicating that the opinions of erperts tended to be consistent.2.Using the Analytic Hierarchy Process(AHP)method to determine the weights of the index system of core competence for nursing professionals with combination of medical and health care According to the scores of the experts on the indicators(Likert five-level scoring method),the AHP method was used to determine the weights of the indicators.3.Reliability and validity test of the index system of core competence for nursing professionals with combination of medical and health care In this study,the project was analyzed by the critical ratio method.The results showed that the determination value(CR)of all items were greater than 3(P<0.01),indicating that the index system had a good degree of discrmination and could detect the degree of reaction of the respondents.All items were therefore retained.In this study,the Cronbach's ? coefficient and split-half confidence were used to test the reliability of the index system.The results showed that the Cronbach's ? coefficient was 0.972,and the split-half confidence was 0.842,indicating that the index system had a good reliability.In this study,the content validity and structural avlidity of the index system were tesed by content validity index and erploratory factor analysis,and the Person conrrelation coefficicent was used to text the correlation among the dimensions of index system and between each dimension and index system.The results showed that the S-CVI was 0.91,which indicated that the content was reasonable.7 common factors were extrated by exploratory factor analysis,and the cumulative variance contribution rate was 63.503%,indicating that the index system had a good structural validity.The correlation coefficient among the dimensions of the index systemscale and the total index system were0.503~0.878(P<0.01),and the correlation coefficirent among dimensions of the index system were 0.167~0.746(P<0.01),indicating that there were correlations and independence between dimensions of the index system and each other.Conclusion1.The Delphi expert interview method was used to screen and revise the index system,and finally formed a index system including 7 first-level indicators,16 second-level indicators and 65 third-level indicators.2.The weights of the index system were determinded.Among the seven firse-level indicators in this study,the weights from large to small were “clinical nursing ability”(0.2987),“health care and old-age care ability”(0.2036),“humanistic quality”(0.1664),“interpersonal skills”(0.1298),“educational and training ability”(0.0843),“professional development ability”(0.0615),“critical thinking ability”(0.0557).3.The reliability and validity of the index system were tested and the results were good.
