[Purpose] Making the survey tool for the willingness to participate in the MTM service,and explore the service model and the factors that influence the willingness to participate in the MTM service.[Methods] Use the literature analysis method and focus group discussion to build theoretical model of the willingness to participate in the MTM service,which is based on perceived value,perceived threat,behavioral restriction,self-efficacy,behavioral clue,willingness and propose the hypothesis.Using the initial questionnaire to conduct the presurvey and modify it according to the feedback of the pre-survey.220 respondents were investigated using the modified questionnaire,and the level of perceived value,perceived threat,behavioral restriction,self-efficacy,behavioral clue and willingness were measured.Using the methods of factor analysis,and Cronbach’s coefficient to modify and adjust the survey tool.Ordinal multinomial logistic regression analysis was performed using measured data to verify whether the theoretical model was reliable and scientific,and whether the hypothesis was established,and explored the factors affecting the willingness to participate in the MTM service.The data was entered and sorted using Excel,and analyzed using SPSS11.0.[Results] 1.After the modification and adjustment of each item,we finally formed the survey tool with perceived value,perceived threat,behavioral restriction,self-efficacy,behavioral clue and willingness.The reliability test results show that the Cronbach coefficient is 0.936,indicating that the internal consistency and stability of the questionnaire are better;the KMO values of each part of the validity test are higher than 0.6,and the overall validity of the final questionnaire is adjusted to KMO = 0.876 which suggests a high validity.2.Descriptive analysis of the questionnaire showed that the perceived value,perceived threat,behavioral restriction,self-efficacy and willingness of patients participating in MTM services were at a higher level.The scoring rate of behavioral restriction was the lowest,69.8% with the mean score of 13.97;followed by the behavioral clue,the scoring rate was 75.5%,with the mean score of 15.11;the willingness,the scoring rate was 75.7% with the mean score of 11.36;the scoring rate of self-efficacy was 77.0% with the mean score of 15.31.The scoring rate of the perceived threat is 78.9% with the mean score of 23.66;the scoring rate of perceived value is relatively high,82.0% with the mean score of 24.60.3.Ordinal multinomial logistic regression analysis showed a perceived threat(β = 0.087,p < 0.05),self-efficacy(β = 0.241,p < 0.000),behavioral clue(β= 0.540,P<0.05),medication for the last three months-no medication(β = 16.655,P < 0.05),medication for the last three months-1~3 kinds(16.487,P<0.05),family annual income-less than 20 thousand(β=-1.281,P<0.05)have significant impact on the willingness to participate in the MTM service.4.The MTM service model shows that the provider of the MTM service is the doctor pharmacist nurse team.People’s ideal MTM service is provided within 15~30 minutes with face-to-face service in the hospital pharmacy,which contains at least personal medication record,medication action plan,medication therapy review.[Conclusions] 1.The survey tool of this study has good reliability and validity,which can reflect he perceived value,perceived threat,behavioral restriction,self-efficacy,behavioral clue and willingness to participate in MTM service reliably and effectively.2.People feel that participating in MTM service brings value;while not participating in MTM service,they feel threatened.And people think the behavioral restrictions as less.3.The increase in perceived threat,self-efficacy and behavioral clue will increase the willingness to participate in the MTM service;people who have not used the drug or have used 1~3 kinds of drug in the past three months are more willing to participate in MTM services;people with family annual income less than 20 thousand aren’t tend to participate in MTM services.4.People’s ideal MTM service is provided within 15~30 minutes with face-to-face service in the hospital pharmacy,which contains at least personal medication record,medication action plan,medication therapy review.
