Objective: We aim to compare the difference between nocturnal and non-nocturnal hypertension,and investigate the relationship between nocturnal hypertension and prethrombotic state and carotid plaques.Methods:From March,2016 to February,2017,142 patients with previously diagnosed primary hypertension were recruited from both inpatient and outpatient clinic of Xiangtan first people’s hospital,and 42 healthy people were recruited from Health Management Center randomly.Age,gender,BMI,platelet count,AT-III,vWF,blood coagulation function(hrombin time,Prothrombin time,Activated partial thromboplastin time,Fibrinogen),D-dimer,Hcy and blood lipid(triglyceride 、 total cholesterol 、 high density lipoprotein-cholesterol 、 low density lipoprotein-cholesterol).24-hour ambulatory blood pressure and the thickness of the Intima-Media Thickness(IMT)of carotid vessels were collected as well.According to the 24-hour ambulatory blood pressure,patients were divided into nocturnal,non-nocturnal hypertension group,and control group.A correlation analysis was conducted among these three groups.Results:1 、 When comparing the experimental group with control group,differences in smoking history,age,systolic pressure,diastolic blood pressure,fasting blood glucose,TG,HDL-c,Hcy,AT-III and Fg were of statistically significant(P<0.05),while there was no statistically significant differences among gender,BMI,TC,TG,LDL-c,vWF,D-dimer,platelet count,TT,APTT,AT-III and Fg(P>0.05).2 、 As for the comparison between nocturnal and non-nocturnal hypertension group,differences in SBP,DBP,TG,Hcy and PT appeared statistically significant(P<0.05),however,no statistically significant differences were discovered between the two groups in gender,BMI,TC,TG,LDL-c,vWF,D-dimer,platelets,TT,APTT,AT-III and Fg(P>0.05).3、A number of 94 plaques were detected among recruited people,and the detection rate showed statistically significant among three groups(P<0.05).Most of the plaques were located in carotid sinus(43.9%),and there were no significant differences between the location of plaques(P>0.05).4 、 Subgroup analysis were carried out among different levels of night-time blood pressure,and the results showed a statistically significant difference among fasting blood glucose,tri-glycerides,high-density lipoprotein cholesterol(HDL-c),Hcy,AT-III and Fg(P<0.05),while there were no differences among BMI,TC,LDL-c,vWF,D-dimer,platelet,PT and APTT(P > 0.05).5、Correlation analysis between mean arterial blood pressure of nocturnal hypertension group 、 non-night nocturnal hypertension and prethrombotic state,the results showed that nocturnal hypertension group was significantpositive correlation with AT-III,and Fg negatively related.6 、 Multivariate logistic regression analysis of nocturnal hypertension showed that smoking,hyperglycemia,high Hcy,Fg anomaly,and plaques might be risk factors contributing to nocturnal hypertension.Conclusion:1、There is a correlation between nocturnal hypertension and prethrombotic status indicators.2、patients with nocturnal hypertension were more likely to develop arterial plaques.3、Concentration of homocysteine was higher in patients with nocturnal hypertension.
