Research On Professor Liu Min’ S Experience In Treating Diabetes Mellitus And Diabetic Peripheral Neuropathy

Objective:DM mellitus,modern medicine called Diabetes mellitus(DM),belongs to the category of common diseases,frequently-occurring diseases and chronic diseases.Ranked third in the global disease killer list after cancer and cardiovascular and cerebrovascular diseases.At present,the incidence of DM is still rising in an unstoppable trend,and the affected population tends to be younger.DM mellitus,the first syndrome of DM,which is namely Diabetic peripheral neuropathy(DPN),with the same trend of disease,seriously affects patients’lives and work.In recent years,Chinese Medicine has obvious advantages in treating DM and its complications.First,the function of the internal organs to regulate the internal organs of the internal organs to reduce and stabilize blood sugar.Even some patients with early-onset DM can be degraded by Western medicine.Second:it can reduce the side effects of western medicine and improve the quality of life of patients.Third:it can give full play to the advantages of Traditional Chinese Medicine in the diagnosis and treatment of DM.Professor Liu Min has been engaged in clinical practice for more than 30 years.He has long devoted himself to the academic thinking and application research of "Treatise on Febrile Diseases".He is good at diagnosing diseases and has accumulated rich clinical experience in the treatment of Chinese Medicine for DM and its complications.The purpose of this research are listed as follows:1.Through literature research to understand the clinical dialectical thinking and experience of some modern Chinese Medicine practitioners in the treatment of DM and DPN,and the progress of western medicine research on DM and snoring to inspire the combination of Traditional Chinese and western medicine.2.Through the collection of articles published by Professor Liu Min and the clinical diagnosis and treatment of DM and diabetic peripheral neuropathy,using data mining methods,combing and summarizing Professor Liu Min under the guidance of the theory of six syndromes to treat DM and The characteristics and ideas of the syndrome of quenching thirst and phlegm,the characteristics of drug use and personal experience.It is hoped that Professor Liu’s experience will be passed on,guiding the later generations of doctors to better diagnose and treat DM and DPN,and establish the confidence of Chinese Medicine in the culture of Chinese Medicine.This has important practical significance for the inheritance and development of Traditional Chinese Medicine in the motherland.Method:The research of this subject included two parts:literature research and clinical retrospective research.Literature research is literature review,which were collected through the database and the library of Guangzhou University of Traditional Chinese Medicine,with the words "DM disease","DM","six syndromes",DM disease","diabetic peripheral neuropathy" and other terms Literature search,screening and collection.This part were divided into Traditional Chinese Medicine research on DM and DPN,and modern medical research on DM mellitus(diabetic peripheral neuropathy).Traditional Chinese Medicine research mainly understands DM and DPN from the perspectives of etiology,pathogenesis,syndrome differentiation and treatment.Modern medical research mainly discusses DPN from several aspects such as epidemiology,pathogenesis,clinical diagnostic criteria and methods,and current clinical treatment methods.The clinical study,the case retrospective study,were collected by photographs from April 20 to December 2018 at the outpatient clinic of the Department of Endocrinology,the First Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine,who had been diagnosed with DM and DPN.The situation included the day’s consultation materials,prescription medications and special medical advice.After arranging the photo data,a total of 646 patients with DM who met the inclusion criteria were screened,and the total number of visits was 1,633.A total of 115 patients with DPN who met the inclusion criteria and 182 effective screenings were screened.In order to better analyze and summarize Professor Liu Min’s clinical experience in the treatment of DM,and to analyze the clinical characteristics of the treatment,especially the experience of medication.All patients were referred to the syndrome of typhoid fever,"the first yin and yang,six dialectical dialectic" and the hospital’s endocrinology clinic treatment specifications,the classification statistics frequency.DM patients were only performed simple statistical processing,focusing on data mining of DPN patients’prescriptions(prescriptions),using double-layer frequency-weighted scissor algorithm,that was,TCM efficacy weight analysis,combined with the usual experience of learning in the clinic.Results:As shown in Table 1,a total of 646 patients with DM were included,and the total number of visits was 1,633.In addition to the six syndromes,there were damp-heat syndrome and blood stasis syndrome.The frequency distribution was from Shaoyang Card(483),Shaoyin Card(470),Taiyin Card(255),Sun Card(255),Yangming Card(58),Yin Yin Card(42),blood stasis.Certificate(31),damp heat certificate(14).As shown in Table 2,Liu Shi’s commonly used prescriptions for treating DM are:Guizhi Fang(40),Bupleurum(40),Zhenwu Decoction(37),Lizhong Decoction(9),Guipi Decoction(9),Si Miao San(5),Yiqi Smart Soup(5),etc.,from the classification of the six classics,mainly focused on sun disease(53),Shaoyang disease(40),Shaoyin disease(39),Taiyin Disease(34),sputum yin disease is rare,almost no Yangming disease(the disease develops to the later stage,yin and yang are damaged).As shown in Table 3,there are 27 commonly used drugs for treating DM in diarrhea,and the drugs in the table were classified.Among them,Guizhi,Zhigancao and Chishao in the Guizhi Tang were used most frequently,followed by Angelica,Astragalus,Aconite,Black Jujube,Red Ginseng,Poria,Atractylodes,Dried Ginger and so on,which were used to nourish qi and nourish the spleen.In addition,they were Achyranthes,Alive,Papaya,Scorpion,Pueraria,Rheumatism,and Meridian;Astragalus,Bupleurum,Pinellia,Codonopsisy keel,which were used to rub the rheumatism through the meridians.Astragalus,Bupleurum,Pinellja,and Codonopsis were used to reconcile Shaoyang and smooth the medicine.Oysters,keels,and jujube kernels were used to collect yin and yang,and to nourish the mind and soothe the nerves;Tianma and Chuanxiong were used to treat blood stasis and relieve pain.Finally,white peony and licorice were used to relieve yin and relieve pain.According to the experience of Liu Shi prescription medication,ginger should also be one of the high-frequency drugs.But because ginger is a fresh product,it was usually a verbal testament.Liu Shihui would tell patients to add appropriate amount of ginger together with peony,so it was not reflected in the table.There were 182 prescriptions in accordance with the requirements of DPN,138 drugs were enrolled,and 97 drugs were summarized.The weight map of the efficacy of prescriptions showed the distribution of the efficacy of Liu Shi in the treatment of high-grade expression of diarrhea.The heat map from deep to shallow included 46 effects,such as Buxin,spleen,qi,pain,drinking,warming,dispelling cold,phlegm.The efficacy network diagram showed a total of 15 groups of efficacy,among them,a93 malaria-a94 insecticide and group 8:a86 treatment of heart palpebral-a97 reclamation and drug network in map group 7 were not strong,the remaining 13 groups are associated with heat map The efficacies indicated are coincident.The drug network diagram showed that the drug-to-large combination had a total of 18 groups,and the small combination had 33 groups.The efficacy between the drug pairs had a corresponding effect,and the 16th group:B4 red ginseng-B39 ginseng were generally different in clinical practice.The method can be used instead of each other.After the combbination of efficacy and drug combination,there were mainly seven groups,1.liver and kidney,convergence and solidification;2.warming in the coke;3.hurricane and collateral;4.qi and collateral;5.cooling blood and eliminating phlegm;6.promoting blood circulation;7.phlegm dampness and water.Each large group is subdivided into corresponding subgroups,a total of 14.There are corresponding representative drugs under each subgroup.See Table 6 for specific drugs.Conelusion:The analysis of the basic side and the analysis of data mining show that Liu Shi’s pathogenesis and treatment of DM mellitus has also evolved from the yin deficiency and heat of diarrhea and qi and yin deficiency.Yin method to Wenyang phlegm and collaterals,and then to Wenyang Yiqi phlegm and collaterals.The representative of high-frequency drugs is essentially the basic application,and the basie party represents the core idea of Liu Shi’s treatment of DM.The qualitative and quantitative study of prescriptions can make up for the deficiency of subjective medication,and new drug pairs can be found through qualitative and quantitative research.For example,the group of Figure 3 of this study and the representative drugs in Table 6 have some new drug pairs,and the efficacy of the drug are highly targeted,and the drug can be used as accurately as possible.This can not only enhance the efficacy of the drug,but also reduce the side effects,adding new vitality and ideas to the traditional prescription drugs.Liu Shi’s treatment of DM and diarrhea are in line with the concept of DM.Follow the book of《Nei jing》"treatment of the disease",make good use of the prescription to help the righteousness,and the prescription are short and fine,the effect is remarkable,one or two of the best.Liu Shi’s use of the effect are concentrated and peaceful,paying attention to the yang of the spleen and kidney of the successive days,and cultivating the original.But he do not stick to the prescription,and will choose the best effective treatment for the patient.Liu Shi also follows Zhongjing’s "viewing his pulse,knowing what is wrong,and treating it with the syndrome",paying attention to the combination of pulse and disease,and treating the disease of DM from two aspects of pulse and disease.On the basis of the basic side,that is,on the basis of the prescription,combined with the pulse disease to add and subtract drugs,according to the changes in the climate of the four seasons and the patient’s physical fitness,the use of relatively peaceful drugs,less use of warm and dry compensatory drugs,pay attention to "to pass the supplement."In the treatment of DM mellitus,Liu Shi made good use of Bupleurum an qi-activating drug,paying attention to the gas lift and the clearance of Sanjiao Waterway in the middle spleen and stomach;During the diagnosis and treatment,the tongue was pressed and the palpation of the abdomen and the yang pulse was emphasized.Asking about sleep and two stools to adjust sleep and spleen and stomach were firstly important.Liu Shi also emphasized the importance of the external treatment method,and self-prepared external washing prescriptions based on Wenyang、Sanhen、Huoxue、Huayu、Tongluo,for patients with clinical manifestations of numbness pain.
