| Gui-zhi Sun is a famous doctor of integrative of tradition and western medicine, who is doctoral and postdoctoral mentor, and national-level professor of traditional Chinese medicine. She has formed a complete set, unique academic thinking system in the combining traditional Chinese and western medicine treatment of tumors from more than 50-yuears clinical experience. For deeply studying the treatment of tumor’s academic thoughts of syndrome differentiation combined with the disease differentiation of the professor Sun, especially in the prevention and treatment of ovarian cancer, now we systematically summarize professor Sun’s academic thought and method, analyze the characteristics of evidence-based medicine and sent the clinical rule, and hope to provide clinical standardized treatment, and further improve the curative effect of beneficial exploration.1 The experience of The professor Gui-zhi Sun in the treatment of ovarian cancer.1.1 Etiology and pathogenesis:The formation of ovarian cancer is because of vital qi deficiency and six evil invading; The damage of spleen and stomach because of improper diet, spleen deficiency generating phlegm, and phlegm together into pieces, then accumulating knot; Emotional disorders, depression of liver qi, qi and blood stasis, pathogenic poison endogenous, all of the above easy to cause blood, phlegm, qi, and wet each knot in the lower abdomen, block channels and collaterals; Or postpartum injury, blood stasis, endogenous qi dysfunction, meridians and uterus is lack of nutrition, generating mass as time passes.1.2 Treatment strategies:(1) Determining etiology and pathogenesis and the pathological parts, strengthening the body resistance and eliminating evil is the principle of treatment. The commonly used formulas are Dan Zhi xiaoyao powder and Chaihushugan powder.(4)The method of invigorating spleen to reduce phlegm:Phlegm and wet are the main pathogenic factors of ovarian cancer, so we need strengthen the spleen and reduce. The spleen-qi is rich, the function of transportation and transformation of spleen is normal, the phlegm and wet will disappear. Commonly used formulas are Erchen decoction and Xiangshaliujunzi decoction.(5)The method of promoting blood circulation and removing blood stasis:Phlegm turbidity, toxic heat, stagnation of qi can lead to blood stasis. The method of activating blood circulation and removing blood stasis very important in the treatment of ovarian cancer. This treatment can treat symptoms such as pain, fever caused by blood stasis, reduce platelet aggregation and viscosity, reduce cancer cells in the blood to stay and grow, in order to prevent the proliferation of tumor cells, transfer function. Commonly used formula is Xuefuzhuyu decoction.1.4 Commonly used prescription(1)Kushen decoction:This is the most commonly used prescription of professor Sun for treating gynecologic malignant tumors, the composition of drugs are Kushen, Ezhu, Tianhuafen. This prescription has the function of clearing heat and nourishing Yin, removing food retention and eliminating stagnation. The drugs in Kushen decoction all have the pharmacological function of anti-tumor, enhance the effect of chemotherapy drug activity.(2)Wuyao powder:With the function of soothing liver-qi and eliminating cold to stop pain. The composition of drugs are Wuyao, Xiaohuixiang, Muxiao and chenpi. The characteristic and flavor of all the drugs are pungent and warm, and belong to the channel of spleen and stomach, therefore the main function is curing the symptoms such as the abdominal distension, abdominal pain, stomachache chills, anorexia and diarrhea which attribute to cold evil invade the body, cold coagulating in meridians and the stagnation of qi.(3) Sanjia decoction:it concludes Guiban, Biejia and shanjia. It has the function of strengthening kidney essence and softing hard lump. It also applies to other malignant tumors, is a broad-spectrum antitumor prescriptions. Sanjia decoction can also prove that professor Sun is good at using animal drugs for the treatment of malignant tumor. Animal drugs, flesh sentient beings, have more advantages in the functions of antitumor and enhancing immunity than the herb drugs.2 Professor Sun treating ovarian cancer clinical observationObjectiveIn the famous doctor of traditional Chinese medicine professor Gui-zhi Sun clinical practice as the foundation, combined with modern advanced data mining technology and scientific testing method, We can take a observation of professor Sun using TCM in the treatment of ovarian cancer, that is for the patient clinical curative effect, the influence of the quality of life, etc. Play the old Chinese medicine experts in a certain role in traditional Chinese medicine academic inheritance. The research plays a certain role of the famous Chinese medicine experts in traditional Chinese medicine academic inheritance. We should inherit and develop traditional academic thinking and valuable clinical experience of the famous Chinese medicine doctor, in order to provide clinical basis for the standardize the application of traditional Chinese medicine treatment of ovarian cancer.Research methodWe retrospectively analyze professor gui-zhi Sun’s 73 patients with ovarian cancer between December 2012 and December 2014, who works in Guang’ anmen hospital China academy of traditional Chinese medicine, then fill in the curative effect evaluation report of combining traditional Chinese and western medicine treatment of ovarian cancer, through the way of telephone follow-up, the basic situation of the research object, survival condition and physical state were retrospectively survey. Finally,through the analysis of the data mining results and combining with the experience of treatment of professor sun,We will discuss the dialectical thoughts and summarize medication regularity.Results1 The basic situation:Through the research about 73 cases of patients with ovarian cancer research, we found that ovarian cancer morbidity is low under the age of 35, and it is concentrated between 56 to 65 years old, gradually declined after 66 years old. Most of he clinical stage is stage III, and the most of the syndrome differentiation type is stagnation of liver qi, and the second type is the deficiency of qi and blood, then the coagulation of phlegm and wetness and qi-stagnancy and blood stasis.2 Symptoms and syndromes analysis:The conclusion of frequency analysis and factor analysis by SPSS17.0 is that symptoms of frequency over 10 is 14, the top five are:Fatigue, abdominal distension less pain, heart palpitations, shortness of breath, modern depression, upset irritability. The main TCM syndrome types of diseases is stagnation of liver qi and qi-blood deficience. A single card type is less and composite card type is major.3 Using drugs regularity:The top ten Chinese herbs in Professor Sun treatment of ovarian cancer are Astragalus, Radix pseudostellariae, Pangolin scales, Oldenlandia, Rhizoma, liquorice, Carapax Trionycis, Rhizoma Smilacis Glabrae, Pollen Typhae, Sculellaria barbata. The basis of common use frequency from more to less were Shiquan Dabu decoction, Sanjia decoction, Erchen decoction, Kushen decoction,Jupi Zhuru decoction, Liuwei dihuang pill.4 Curative effect evaluation:the total effective rate of the type of stagnation of liver qi is 80.0%; The total effective rate of the type of the deficiency of qi and blood is 68.0%; The total effective rate of the type of the coagulation of phlegm and wetness is 53.8%; And the total effective rate of the type of qi-stagnancy and blood stasis is 40.0%. After taking professor Sun’s TCM decoction, KPS score of all the patients uplift ratio and index of stability is 82.2%.Research conclusion1 The result reveals the susceptible age of ovarian cancer is over 60 years old, and has positive significance about the prevention and screening of the ovarian cancer. These results also provide a reliable basis for the traditional Chinese medicine treatment of ovarian cancer patients with different age groups.2 The main TCM syndrome types of diseases is stagnation of liver qi and qi-blood deficience. This result can more objectively reflect the ovarian cancer patients with TCM syndrome type distribution characteristics, and provides favorable evidence for the traditional Chinese medicine treatment of ovarian cancer. The results suggest the tumor of the complexity of the condition, deformation, affirmation of the advantage of combining traditional Chinese and western medicine treatment of the disease.3 From the statistics of commonly used drugs and basic prescription, we find that the following treatment of remedying ovarian cancer are the most often used by teacher,including invigorating Qi and nourishing blood, strengthening spleen and nourishing kidney, clearing heat and removing toxicity, resolving hard lump. In the whole process of treatment of ovarian cancer, professor sun always supporting the application of strengthening the body resistance and eliminating evil in TCM.4 According to the result of curative effect evaluation, can draw the following conclusions:Integrative Chinesewestern medicine can enhance the effect of chemotherapy drugs, and reduce the side reaction of chemotherapy. Some TCM treatment is reasonable and effective for ovarian cancer, such as strengthening the body resistance and eliminating evil, clearing heat and removing toxicity, dispersing the stagnated liver qi, invigorating spleen to reduce phlegm, promoting circulation and removing stasis. |
