Objective:To evaluate the value of narrow-band imaging in the early diagnosis of nasopharyngeal carcinomaMethod:112 patients with suspected nasopharyngeal carcinoma who were admitted to the otolaryngology department of the First Affiliated Hospital of Bengbu Medical College from September 2016 to March 2019 were selected.Traditional white endoscopy and narrow-band imaging mode endoscopy were used to observe the patients,and benign lesions and malignant lesions were predicted according to their respective microscopic features,and then local biopsy was performed.The pathological diagnosis was based on the gold standard.The contrast sensitivity,specificity,coincidence rate,positive predictive value,and negative predictive value were compared.The chi-square test was performed.When p<0.05,the difference between the two groups was considered statistically significant.This is to evaluate the ability of traditional white endoscopy and narrow-band imaging endoscopy in the diagnosis of nasopharyngeal carcinoma.Result:Among the 112 patients: 47 cases of benign nasopharyngeal lesions,4 cases of NBI positive,43 cases of NBI negative,the positive rate was 8.5%,the specificity was 91.5%.Among 65 cases of nasopharyngeal carcinoma,NBI was positive in 54 cases and NBI was negative in 11 cases.The sensitivity was 83.1%,the coincidence rate was 86.6%,the positive predictive value was 93.1%,and the negative predictive value was 79.6%.Under ordinary white light microscope,47 cases were benign nasopharynx.Among the patients with lesions,41 were negative for white light and 6 were positive,with a positive rate of12.8% and a specificity of 87.2%.Of the 65 patients with nasopharyngeal carcinoma,44 were positive for white light and 21 were negative.The sensitivity was 67.7%,the coincidence rate was 75.9%,the positive predictive value was 88.0%,and the negativepredictive value was 66.1%.NBI is more advantageous in the diagnosis of nasopharyngeal carcinoma than traditional white light microscope in sensitivity and coincidence rate(sensitivity: 83.1%>67.7%,2=4.145,p value 0.042<0.05;coincidence rate: 86.6%>75.9%,2 = 4.220,p value is 0.040 < 0.05).Conclusion:Compared with traditional white light endoscope,narrow-band imaging technology can better identify the microvessel morphology on the mucosal surface,and has obvious advantages in sensitivity and compliance rate.It can guide the selection of biopsy site and improve the positive detection rate and diagnosis rate.
