Phenomenological Research Of Night Shift Communication Between Doctor And Nurse In General Wards

Objectives:From the both side from doctors and nurses,this paper uses descriptive phenomenological method in qualitative to explore the general phenomena and communication experience of night shift doctors and nurses,in order to explore the problems and related cultural theories in night shift doctors and nurses communication,and to provide better reference for night shift nurses and managers on how to improve the quality of night shift communication and the efficiency of night shift doctors and nurses communication.Design Based on the non-participatory clinical observation,the objective sampling snowball sampling method was used to descriptive phenomenology interview subjects.Non-participatory clinical observation combined with phenomenological analysis method is used in this paper.Data analysis was carried out by Colaizzi data analysis method and MAXQDA12 qualitative software.Results:The transcription data of qualitative research were transcribed word by word,and the mood,intonation and expression of the interviewees were recorded.After proofreading by two people,the transcription documents consisted of 50 pages and 69743 words.Through qualitative software MAXQDA12 combined with Colaizzi method,the same problem between doctors and nurses was studied and analyzed from different angles(doctor referred to as "D" and nurses refcrred to as "N").In the analysis of qualitative software,375 first-level codes are finally established.42 second-level codes are formed by tree structure analysis,fusion,comparison and deletion.Thirteen third-level codes are formed by merging.Finally,four fourth-level codes are formed by reading literature and drawing up by experts.Now the theme code are as follows:(1)Night shift medical and nursing communication pays attention to the realization of the goal:night shift communication as a target tool in general ward,mainly to solve the patient's condition as the center,to improve the efficiency of night shift work as the goal.Night shift medical communication in general ward is a supportive and comfort tool.The study found that there are specific content needs,timing needs and process needs for night shift medical and nursing communication.(2)There is no absolute ideal state in general ward's night-shift medical and nursing communication:psychological pressure,cognitive differences,ineffective state and other obstacles in night-shift medical and nursing communication,at the same time,there are stage,group,seniority and time differences in night-shift medical and nursing communication.(3)There is heavy relationship culture in general ward's night shift communication:control ability of good relationship;intimacy of good relationship;instantaneity of good relationship;respect of good relationship(4)Night shift medical and nursing communication in general ward is learnable ability:learning of night shift medical and nursing communication should be specifically divided into communication preparation;understanding in communication;growth after communication;growth after obstacles.Conclusions:Hospital managers should pay attention to the particularity and quality of night shift communication between doctors and nurses.There is no absolute ideal state of night shift communication between doctors and nurses in general ward.There are some obstacles in night shift communication,such as psychological pressure,cognitive difference,emotional fluctuation and so on.And also should pay attention to the prevention and solution of obstacles,knowing specific process,timing and content needs of goal realization,to improve the efficiency of night shift medical and nursing communication.At the same time,we should respect differences and pay attention to the differences of stages,groups,seniority and time in general ward's night shift communication.Only by examining the culture of medical-nursing relationship existing in night shift doctor-nursing communication and making rational use of it,through reasonable training and education,can we improve the efficiency of night shift medical-nursing communication and guarantee the quality of night shift nursing work.Night shift communication in general ward is a interactive behavior,and it is a dynamic and continuous learning process.learning to prepare for communication,understand in communication,grow after communication,and deal with obstacles.In a word,general ward's night-shift doctor and nurse communication is guided by the efficiency of achieving the goal of night-shift communication.In order to improve the efficiency of night-shift medical and nursing communication and nursing work efficiency,we should pay attention to the hidden obstacles behind the general ward's night-shift doctor and nurse communication,and the relationship culture among them,learn the specific communication skills and methods in the whole stage.
