| ObjectiveTo investigate the status of perceived controlling of Ischemic Stroke patients and to further explore its related factors;To analyze the correlation among the perceived controlling of disease,social support and quality of life;To determine whether the social support has a meditating effect between the effect of perceived control and patients’ life quality.The purpose of this study is to raise the awareness of medical staff to assessing the level of disease perceived control in Ischemic Stroke population.This study aims to provide a theoretical basis for establishing continuous nursing intervention mechanism and carrying out the individualized health education.MethodsThe cross-sectional study and convenience sampling method have been applied in this research,from October 2017 to August 2018,243 ischemic stroke patients were selected from emergency department,neurology and the getiatric ward of two Grade A hospitals in Zhengzhou.The general questionnaires,Multidimensional Health Locus Of Control Scale(MHLC-C),Social Support Rating Scale(SSRS)and Stroke Speciality-Quality of Life(SS-QOL)were used to conduct surveys on the ischemic stroke patients.Epidata 3.1 software was used to establish the database and SPSS20.0 software was used to analyze the data.Descriptive statistics were made with frequency,percentage description,mean±standard deviation.The independent samples t-test and one-way analysis of variance were used for comparison between groups.Correlation analysis was used for Pearson correlation was used for analyze the correlation between perceived control,social support and quality of life,,multivariate linear regression was used to analyze the correlation between perceived control and other variables.Results1.The status of perceived control of patients with ischemic strokeThe score of internal control(22.02 ±8.577),lower than that of norm;external authority control score of(25.45 ±6.597),higher than that of norm;opportunity control score of(20.47 ±11.671),higher than that of norm.The control score of external authority control was higher than others.According to the score rule of master scale,ischemic stroke patients prefer to be controlled by external authorities in hospital.2.The status of social support and life quality in ischemic stroke patientsThe total score of social support was(33.04±7.369),which is generally at a medium level.The subjective support dimension score was(7.55±2.649),The objective support dimension score was(19.98±4.846),and the support usability dimension score was(5.48±1.855).Social support is generally at a medium level.The total score of life quality was(160.92±37.471),and such the score is low.3.The single factor analysis of general information on ischemic stroke patients with perceived control.There was a statistically significant difference in the internal control among gender,age,educational level,family income,household registration,marriage,work,re-admission,number of hospital admission,course of disease,number of complications,number of personal history,BI;There was a statistically significant difference in the external authority control among gender,marriage,re-admission,number of hospital stay and course of disease;There was a statistically significant difference in the opportunity control among gender,education level,family income,household registration,marriage,work,re-admission,number of hospital stay,course of disease,number of complications,number of personal history,family history,treatment,BI.4.Relationship between perceived control,social support,life quality among the ischemic stroke patients.The results showed that there was positive correlation between the level of social support and internal control and the control of external authority,and the correlation coefficient was r = 0.643,r = 0.377(p <0.05).The correlation coefficient was r =-0.790(p <0.05).The quality of life of patients with ischemic stroke was positively correlated with internal control and external authority,and the correlation coefficient was r = 0.722,r = 0.254(p <0.05).The correlation coefficient is r =-0.850(p <0.05).5.multivariate linear regression analysis of factors affecting ischemic stroke patients with perceived control.The results of multiple linear regression analysis showed that gender,age,education level,household registration,marriage,course of disease,Barthel index,social support were involved in the internal control regression equation,explain 62.1% of the total variation.The gender,marriage and social support entered the external authority-controlled regression equation,explained 20.1% of the total variation.The household registration,Barthel index,number of complications,personal habits,family history,social support entered the opportunity controlled regression equation,explained 74.9% of the total variation.6.Social support of ischemic stroke patients with perceived control in the analysis of the mediating effect between perceived control and life quality.The mediating effect analysis used sequential tests.The results showed that there are partial mediating effects between health internal control and quality of life,and the mediating effect accounted for 51.2% of the total effects;There is a complete intermediary effect between health authority control and quality of life;and a partial intermediary effect between health opportunity control and quality of life,which can explain 32.1% of the total effect.Conclusions1.The perceived control of patients with ischemic stroke is more likely to be dominated by external authorities control,with lower internal control level and higher level of opportunity control.2.The internal control of patients with ischemic stroke,external authority control and social support,quality of life are positively related,and the control of opportunity is negatively related to social support and quality of life.3.The factors influencing the internal control of the patient include gender,age,education level,household registration,marriage,course of disease,Barthel index,social support;the influential factors of the external authority control of the patient include gender,marriage and social support;the influencing factors of opportunity control include household registration,Barthel index,number of complications,personal habits,family history,social support.4.The social support level has a partial intermediate effect between the disease-aware control and the quality of life. |
