Part Ⅰ Study of digital lung three-dimensional imaging technology combined with pulmonary function in early COPD screeningObjective To study the methods of assessing small airway disease and screening early COPD by digital lung three-dimensional imaging technology combined with pulmonary function.Methods The study included 136 patients who underwent pulmonary function(PFT)at the 903 Hospital of the People’s Liberation Army from January 2018 to January 2019.According to the PFT results,the enrolled patients were divided into two groups:(1)PFT suggested small airway airflow limited,and FEV1 / FVC > 70% for the observation group,a total of 90 cases;(2)PFT tipped: FEV1 / FVC > 70%,but there is basically no airflow limitation were in the control group,a total of 46 cases.The observation group and control group patients completed the CT respiration biphasic scan within 1 week,and the image was input into the digital lung three-dimensional imaging technology post-processing software,to calculate the measurements indicators of the digital lung three-dimensional imaging technology,such as small airway lesion area(PRM f SAD)、emphysema area(PRM Emph)、lung volume(LV)、low-attenuation area of the lung with attenuation values below-950 Hounsfield units(LAA950%),and wall thickness(WT)、lumen diameter(LD)、wall thickness to diameter ratio(TDR)、wall area percentage(WA%)、lumen area(LA)of the 5-7 grade bronchus of RB1、LB1、RB10 and LB10 category.Using SPSS19.0 software to observe the measurements indicators of digital lung three-dimensional imaging technology and PFT of the observation group and control group,and the results expressed by (?) ± SD.When P < 0.05,the difference was statistically significant.Result 1.The patient’s general information: the age、sex ratio、smoking composition ratio and body mass index of observation group and control group were no statistically significant difference(P>0.05).2.Correlation analysis between measurement index of digital lung three-dimensional imaging technology and PFT index:(1)PRM f SAD、PRM Emph、LV、LAA950%、WT、TDR、WA%of 5-7 grade and FEV1、FEV1%、FEV1/FVC、MEF25-75、MEF25、MEF50、MEF75、PEF were all negatively correlated(P < 0.05);PRM Emph、LV、LAA950%,TDR、WA% of 5-6 grade,WA% of 7 grade,and DLCO were negatively correlated(P < 0.05).(2)LD、LA of 5-7 grade and FEV1、FEV1%、FEV1/FVC、MEF25-75、MEF25、MEF50、MEF75、PEF、DLCO were positively correlated(P < 0.05),prompting lung three-dimensional digital imaging technology could effectively evaluate the small airway function changes.3.The comparison of PFT indicators between the observation group and control group: There were significant differences between the observation group and control group of FEV1、FEV1%、FEV1/FVC、MEF25-75、MEF25、MEF50、MEF75、PEF and DLCO(P<0.05).4.The comparison of the digital lung three-dimensional imaging technology between the observation group and control group: there were significant difference between the observation group and control group of PRM f SAD、PRM Emph、LV、LAA950%、WT、TDR、WA%、LD、LA of 5-7 grade(P < 0.05).Conclusion 1.Digital lung three-dimensional imaging technology measurement indicators can effectively evaluate the small airway function disease.2.Digital lung three-dimensional imaging technology combined with PFT can screen early COPD,To early individualized treatment.Part Ⅱ Study of digital lung three-dimensional imaging technology assessment drug curative effect of early COPDObjective To research the value of digital lung three-dimensional imaging technology assessment of drugs in the therapeutic value of early COPD and guide individualized treatment.Methods According to digital lung three-dimensional imaging technology combined with PFT to screen early COPD patients,which selected 42 patients with clinical symptoms from(such as cough、sputum、chest tightness,etc.):(1)the control group(before drug therapy);(2)the observation group(after drug treatment).All patients were given inhalation with budesonide suspension(Pulmicort)2mg + terbutaline sulphate atomization solution(Bliconi)5mg nebulized 3 times per day for 7 days;then budesonide Formoterol powder inhaler(160ug)1 inhalation 2 times per day for a total of 21 days.After 1 month of drug treatment,the CT respiration biphasic scan and the PFT examination were completed again,The CT image was input into the digital lung three-dimensional imaging technology post-processing software,to calculate the measurements indicators of the digital lung three-dimensional imaging technology,such as PRM f SAD、PRM Emph、LV、LAA950%,and WT、LD、TDR、WA%、LAof the 5-7 grade bronchus of RB1、LB1、RB10 and LB10 category.The measurements indicators of the PFT are FEV1、FEV1%、FEV1/FVC、MEF25-75、MEF25、MEF50、MEF75、PEF、DLCO.Using SPSS19.0 software to observe the measurements indicators of digital lung three-dimensional imaging technology and PFT of the observation group and control group,and the results expressed by (?)± SD.When P < 0.05,the difference was Emph、LV、LAA 950%,and WT 、TDR、WA% of 5-7 grade indicators were decreased and significantly different in the observation group(P < 0.05).(2)The LD and LA of 5-7 grade index were increased and the difference was significant(P < 0.05).3.The comparison of PFT indicators between observation group and control group: Compared with the control group,the FEV1、FEV1%、FEV1/FVC、MEF25–75、MEF25、MEF50、MEF75、PEF,and DLCO indexes were increased and significantly different in the observation group(P < 0.05).Conclusion 1.Early drug therapy can effectively improve the small airway disease in early COPD patients.2.Digital lung three-dimensional imaging technology combined with PFT could effectively evaluate the therapeutic value of drugs in early COPD,guide individualized treatment.
