Purposes:To observe the curative effect of external application of ChuangLing Ye in the treatment of non-puerperal mastitis(NPM)wounds,and to explore the effective mechanism,in order to promote the use of a reliable basis for clinical improvement:the use of randomization.Methods:Using the method of random parallel control,a total of 45 patients were included in the experiment.According to the condition,there were 23 patients in the control group,2 in the healing group and 3 in the shedding group,and 22 in the experimental group,4 in the healing group and 5 in the shedding group.Control group:on the basis of basic treatment(traditional Chinese medicine decoction treatment and diet treatment),according to the patient’s condition,the patients were given traditional Chinese medicine surgical dressing changes,including abscess incision and drainage or puncture and drainage,saline irrigation of the wound cavity,and wound drainage with scutellaria oil gauze.Fresh granulation tissue specimens of the patients in the group were cut and taken in the process of dressing change the bandage on alternate days(D1,D14,D28).The specimens were cut into strips and sealed in a sterile 3ml frozen tube.The specimens were stored in a liquid nitrogen tank and transported to the laboratory to make homogenate.Treatment group:on the basis of dressing change in the control group,add ChuangLing Ye for external treatment.The specific method was to use 30ml ChuangLing Ye for washing,and use the yarn with ChuangLing Ye for drainage and dressing change twice a day.Patients in the two groups were observed alternate days after dressing change.During the treatment,the size of the breast mass,the pain,the percolation and the local IL-6 CD20 CD68 level in the wound were detected,and statistical treatment was performed.Results:Basic conditions of subjects in the two groups included in this study showed no significant difference,which was comparable(P>0.05).1.Mass size:there was no significant difference in mass size between the two groups before treatment(P>0.05),which was comparable.There was an interaction between time and treatment,F(2,38)=3.3 17,P=0.047<0.05.There was a statistical difference between the two groups after treatment,P=0.001 was statistically significant.2.Breast pain:there was no significant difference in breast pain before treatment between the two groups(P>0.05),which was comparable.There was an interaction between time and treatment,F(2,38)=4.795,P =0.014<0.05.There was statistical difference between the two groups after treatment,P=0.001 was statistically significant.3.Wound seepage:there was no significant difference in the number of wound gauze permeable layers between the two groups before treatment(P>0.05),which was comparable.There was an interaction between time and treatment,F(2,38)=4.795,P=0.867>0.05.The P value of the main effect was 0.53(P>0.05),and the difference was not statistically significant.4.Local IL-6 level on the wound:there was no significant difference in local IL-6 level on the wound between the two groups before treatment(P>0.05),which was comparable.Compared within the group,the local IL-6 level in the wounds of the two groups decreased after treatment(P<0.05),which was statistically significant.The intergroup comparison showed that the decrease of local IL-6 level in the wounds of the experimental group was significantly better than that of the control group(P<0.05),which was statistically significant.5.Local CD20 CD68 level of wound:there was no significant difference in the level of local immune index of wound between the two groups before treatment(P>0.05),which was comparable.Intra-group comparison showed that the level of local immune indexes of wound in both groups decreased after treatment(P<0.05),which was statistically significant.Compared with the control group,the decreased level of local immune indexes in the experimental group was significantly better than that in the control group(P<0.05),with statistical significance.Conclusions:Topical dressing change of ChuangLing Ye in the treatment of non-lactating mastitis patients can significantly shrink the breast lump and relieve pain by adjusting the local immune response on the wound.The down-regulation of local IL-6 CD20 and CD68 on the wound is effective and safe. |
