Objective:To relieve pain the most significant clinical symptom in patients with KOA by using the TCM external application technology of waist and knee;And according to the research on the clinical effect of treating KOA with the technology,to expound its mechanism and form a new treatment method.Methods:48 cases were accordance with the inclusion criteria,and according to the different drug sites of Layers Adjusting External Application(LAEA)were randomly divided into 3 groups.The waist and knee co-dressing group was treated by applying LAEA to the waist and knee.The waist dressing group was treated by only applying LAEA to the waist.The knee dressing group was treated by only applying LAEA to the knee.Patients received treatments for 8 weeks.The WOMAC and VAS scores were used as the observations to evaluate the efficacy in 6 different time node:before-therapy,2week post-therapy,4week post-therapy,6week post-therapy,8week post-therapy and lmonth post-therapy.Results:1,Baseline data among with the three groups before treatment are similar(p>0.05).2,There was statistically difference between the comparison of 2week post-therapy,4week post-therapy,6week post-therapy,8week post-therapy,lmonth post-therapy and before-therapy in various points of WOMAC and VAS(p<0.05),indicating that three groups of treatment for KOA all have curative effect.3,At 2week post-therapy,there was statistically difference between the waist and knee co-dressing group compared to the knee dressing group and the waist dressing group in WOMAC pain scores,WOMAC physical function scores and VAS scores(p<0.05).While in WOAMC stiffness scores,the waist and knee co-dressing group only have statistically difference compared to the waist dressing group(p<0.05).The effective time of the waist and knee co-dressing group is shorter than the other two groups.4,At 4week post-therapy,there was statistically difference between the waist dressing group compared to the other two groups in WOMAC pain scores,WOMAC physical function scores and VAS scores(p<0.05).The effective time of the waist dressing group in pain and physical function is longer than the other two groups.5,AT 6week post-therapy,8week post-therapy and lmonth post-therapy,there was no significant difference among with the three groups(p>0.05).The long-term efficacy is equivalent to the three methods.6,After the end of the last treatment,there was no significant difference in efficacy rate among with the three groups(p>0.05).But the three groups showed difference in efficiency rate,the difference was statistically significant(p<0.05),the efficiency of the waist and knee co-dressing group was73.3%,significantly higher than the waist dressing group which showed a efficiency of 25%(p<0.05).The knee dressing group showed a efficiency of 56.3%lower than the waist and knee co-dressing group,but there is no statistically significant difference between the two groups(p>0.05).Indicating that the three groups of treatment for KOA all have curative effect.The waist and knee co-dressing group is more effective than the waist dressing group.Conclusion:LAEA is safety and applying LAEA to waist,knee or both waist and knee are all effective to improve the KOA patients’ joint pain and life quality.Nevertheless applying LAEA to both waist and knee is better efficacy and shows significant advantage of shorter effective time.As far as the current experimental data are concerned,the TCM external treatment technology of waist and knee can be a new method to treat KOA.
