Objective:1.To prove that Yiceng adhesive therapy can improve cold,hot and mechanical pain threshold in patients with knee osteoarthritis,relieve pain symptoms in knee osteoarthritis patients,improve knee joint function and improve their quality of life.2.From the perspective of evidence-based medicine,the good clinical effect of external patches of traditional Chinese medicine on patients with knee osteoarthritis was expounded.Methods:In this study,40 patients with KOA were randomly divided into treatment group and control group,20 patients in each group,in which the treatment group was applied with Yiceng and the control group was applied with Voltaren Emulgel.The treatment course of the two groups was 4 weeks,and the WOMAC score and VAS score were performed before and after the treatment,the cold and hot pain threshold and mechanical pain threshold were measured,and the clinical efficacy was observed and evaluated;2.Search CNKI,VIP,Wanfang Data,CBM,PubMed,EMBASE and web of science,all of them the retrieval time of the database is from the establishment to October 2019.In the randomized controlled trial,the control group was treated with Voltaren Emulgel,while the experimental group was treated with traditional Chinese medicine patch.Two evaluators independently extracted data and evaluated the quality of literature,using Revman 5.3 software for meta analysis.Results:1.There was no statistical difference between the two groups in terms of demographic data;2.Before treatment,there was no statistical difference between the experimental group and the control group in the measurement of mechanical pain threshold,cold pain threshold and hot pain threshold in different parts of the affected knee(P>0.05),and the two groups were comparable;2.After 4 weeks of treatment,the experimental group compared with the control group in the measurement of mechanical pain threshold,cold pain threshold and hot pain threshold in different parts of the affected knee Results there was no significant difference between the two groups(P>0.05);? after 4 weeks of treatment,there was a significant difference value between the experimental group and the control group in the pressure pain threshold of inner knee eye,outer knee eye,outer joint space and goose foot,in the cold pain threshold of outer knee eye(P<0.05),in the pressure pain threshold of inner joint space and suprapatellar capsule,and in the thermal pain threshold of outer knee eye Results there was no statistical difference value(P>0.05);?before and after treatment,there was a statistical difference(P<0.05)in the control group in the measurement of pressure pain threshold of the internal knee joint space and the suprapatellar capsule;there was no statistical difference in the measurement of pressure pain threshold of the internal knee eye,the external knee eye,the external joint space and the goose foot,etc.;? there was no statistical difference in the test group in the measurement of cold and hot pain threshold of the external knee eye(P>0.05);? Before and after treatment,the pressure pain threshold of inner knee eye,outer knee eye,outer joint space,goose foot and suprapatellar bursa,and the cold pain threshold of outer knee eye had statistical difference(P<0.05).There was no statistical difference in the pressure pain threshold of inner knee joint space and the thermal pain threshold of outer knee eye(P>0.05);3.? before treatment,the pain,stiffness and difficulty of knee joint between the two groups There was no significant difference between the two groups(P>0.05).? After 4 weeks of treatment,compared with the control group,the test group had statistical differences in pain,stiffness score and the total score of WOMAC(P<0.05),but no statistical differences in difficulty score(P>0.05);?before and after treatment,the control group had no statistical differences in pain,stiffness,difficulty and the total score of WOMAC(P>0.05);?before treatment,the test group had no statistical differences There were significant differences in pain,stiffness,difficulty degree and total WOMAC scores between the two groups(P>0.05).4.After the treatment,there were significant differences in the efficacy distribution between the"easy layer" application group and the Futalin emulsion group,and the "easy layer" application treatment koa was better than the Futalin emulsion(X2=8.286,P=0.04);5.Meta analysis showed that the external application of traditional Chinese medicine was better than the external application of Futalin emulsion(X2=0.16,P=0.09);5 The total effective rate of symptomatic improvement of knee osteoarthritis was better than that of the control group,with statistical difference[OR=4.18,95%CI(2.88,6.05),P<0.00001];5.Meta analysis results showed that in terms of WOMAC score,traditional Chinese medicine external patch was better than that of the control group,with statistical difference[MD=-5.82,95%CI(-8.41,-3.23),-P<0.0001];Meta analysis results showed that traditional Chinese medicine external patch was better than that of the control group In terms of VAS score,the therapeutic effect of traditional Chinese medicine on knee osteoarthritis is better than that of the control group,with statistical difference[OR=-0.91,95%CI(-1.60,-0.23),P=0.009];6.Meta analysis results show that the safety of external application of traditional Chinese medicine on knee osteoarthritis is similar to that of the control group,without statistical difference[OR=1.34,95%Cl(0.49,3.66),P=0.57].Conclusion:1.Yiceng therapy can effectively improve the knee pain symptoms of patients with knee osteoarthritis,improve the mechanical pain threshold and cold pain threshold of patients with knee joint,improve the quality of life of patients,and have good safety.2.From the perspective of evidence-based medicine,it is proved that external application of traditional Chinese medicine patch has a good effect on the treatment of knee osteoarthritis.
