Purpose: This study investigates the status of ICU nurses' professional autonomy and nursing work environment in tertiary level 1st class general hospitals in Jilin Province,analyzes the impact of demographic data on nurses' professional autonomy and nursing environment,and explores the relationship between ICU nurses' professional autonomy and nursing work environment.It provides a theoretical basis for improving the professional autonomy of ICU nurses and improving the nursing work environment,and provides reference for nurse managers to make relevant decisions.Method: Using the convenience sampling method,320 ICU nurses were selected as the study subjects from eight tertiary level 1st class general hospitals in Jilin Province.Nurses' general situation questionnaires,nurses' professional autonomy scales and nursing work environment scales were used as research tools to carry out questionnaire surveys and SPSS 23.0 software was applied to perform t-test,variance analysis,rank sum test and Pearson correlation analysis and multiple stepwise regression analysis.Result: The level of professional autonomy of the surveyed ICU nurses was good,with the highest score of work attitude,and the lowest score of social recognition for nursing profession.The scores of professional autonomy of nurses with different ages,marital status,final academic qualifications,working years,job titles,nurse hierarchies,employment patterns,rotational night shifts,and incomes were statistically significant(P<0.05).ICU nurses had a good perception of the nursing work environment,and high-quality nursing service was the highest score,and the lowest score of adequate manpower and material resources.The scores of nurses' nursing work environment with different marital status,final education,nurse level,employment form,and income were significantly different(P<0.05).The total score of nurses' professional autonomy was positively correlated with the nursing work environment.(P<0.001).The multivariate stepwise regression analysis results of the total professional autonomy of nurses showed that the final education level,job title,ability and leadership style of nursing managers,sufficient human and material resources,and high-quality nursing service basis,five variables entered the regression equation(P< 0.05).Conclusion: The ICU nurses in tertiary level 1st class general hospitals in Jilin Province have good professional autonomy and are relatively satisfied with the nursing work environment.Nurses' professional autonomy is positively correlated with nursing environment.Factors that can affect the overall level of professional autonomy of nurses in the ICU include: final education,job title,and nursing Manager's ability and leadership style,sufficient human and material resources,and high quality care service foundation.
